Abstract
Since the first endovascular repair of abdominal aortic aneurysm was reported in the early 1990s, there has been an explosion in the volume and complexity of endovascular and hybrid procedures for the treatment of aortic diseases. Endovascular techniques and technologies have evolved from the initial devices that allowed treatment of only the most straightforward infrarenal aneurysms with appropriately long and straight necks to the scenario that pertains today, wherein a customized endovascular solution is almost always available for even the most complex aortic aneurysm. There is growing evidence in favor of the endovascular treatment of these aneurysms, which extend above the visceral aortic segment, as well as for the treatment of thoracic aortic dissection and aneurysm. These new endoaortic surgical procedures have been proven to shorten hospitalization, reduce morbidity and mortality, speed recovery, and hasten return to normal life, while their evolution and conceptual design would obviously not be possible without the simultaneous explosion in medical imaging technologies that has taken place over a similar time period. The Atlas of Advanced Endoaortic Surgery has been compiled to engage its readers in a comprehensive yet concise manner, with numerous illustrations of real clinical cases. It provides a useful tool for practitioners as they plan and execute treatment of patients with these various aortic pathologies, and serves as a useful reference as this segment of the field continues to evolve.

Currently, such skills are commonly developed by the empirical trial and error method only. In this textbook, for the first time, an explicit teaching, training, and learning approach is set out that will enable interventional operators, whether cardiologists, vascular surgeons, vascular specialists, or radiologists, to learn about and to develop the cognitive skills required in order to achieve consistent expert-level catheter-based interventions. It is anticipated that adoption of this approach will allow catheter-based interventions to become a domain of excellence, with rapid transfer of knowledge, steep learning curves, and highly efficient acquisition of complex skills by individual operators' all of which are essential if we are to meet successfully the challenges of modern cardiovascular care embedded in highly competitive health care markets.

This is the first textbook entirely dedicated to the endovascular treatment of complications related to arteriovenous accesses for hemodialysis (native fistulas and prosthetic grafts). The book addresses the anatomy of upper limb arteries and veins as well as the clinical presentation of patients and the way to perform and read a fistulogram. Many details and illustrations are provided, clarifying the subtleties in catheterization and the dilation of stenoses. From a strategic point of view, it is explained for the first time that many stenoses must either be ignored or deliberately underdilated. Many images help understand the different stages of recovery from thrombosed accesses. The number of incident dialysis patients increases by 5 % every year, which means that this is a field of growing interest. Endovascular neurosurgery is a recently introduced but rapidly evolving medical field, which uses minimally invasive interventions to treat major life-threatening vascular lesions of the Central Nervous System. Although its history counts less than 15 years of worldwide acceptance, it has rapidly displaced the traditional open neurosurgical techniques, being nowadays the first treatment choice for brain aneurysms and vascular malformations. Thus, the experience of each neuroendovascular center and performer is invaluable, offering the base for learning and teaching the new generation of interventionalists as well as for the evolvement of the method itself. This book presents the basic principles of endovascular neurosurgery starting from clinical cases. Through this close-to-clinical-reality-process, the reader will be able to more thoroughly understand the pathophysiology of the brain and spine vascular lesions as well as the decision-making strategy, related to the indications, endovascular methods and results, finding suggestions and solutions to his/her clinical questions and problems. Besides chapters devoted to CNS vascular embryology and anatomy, clinical cases organized in groups based on the treated lesions are introduced: ruptured and unruptured cerebral aneurysms of the anterior and posterior circulation, side-wall and bifurcation aneurysms, arteriovenous malformations (AVM), dural arteriovenous fistulae (dAVF), arterial stenosis and angioplasty as well as spinal vascular lesions. A separate chapter is devoted to the organization and necessary equipment of the angio room and the department offering neuroendovascular service. This volume will be of interest to neurosurgeons, interventional neuroradiologists, vascular surgeons, neurologists and ICU physicians as well as health care providers who are involved in the diagnosis and management of the vascular lesions of the brain and spine.
