Abstract
Focusing exclusively on the older patient with poor bone quality, this unique book presents the indications, contraindications and common techniques, as well as the risks, benefits and outcomes—for utilizing arthroplasty for the treatment of fractures in this population, sensibly divided into four thematic sections. The incidence and burden of fragility fractures is presented in part one, with considerations of the influences of osteoporosis on both treatment and healing. The subsequent three sections cover upper extremity fractures, lower extremity fractures, and peri-articular nonunions, respectively. However, far from providing a simple "how-to" for the techniques discussed, the relevant current literature is presented as well as the common techniques employed in fracture management, allowing the reader to select the best approach for the specific patient presentation. In addition, chapters are included here that cover degenerative joint conditions not typically treated with total joint replacement. The treatment of fragility fractures is constantly evolving, and the aging population is consistently expanding, creating a strong need for clinicians who have experience with and exposure to the use of arthroplasty techniques as an option in their successful treatment. Written and edited by leaders in the field, Arthroplasty for the Treatment of Fractures in the Older Patient is an invaluable resource for orthopedic surgeons, residents and support staff who see and treat these increasingly common injuries.

Abstract
This book focuses specifically on the management of cardiovascular disease in elderly patients with particular frailties and in the "oldest old". It is clearly explained how treatment in these patients differs from that in patients in their seventies who respond well to therapies and do not present frailties or organ failures. Although not young, the latter patients can be treated almost like any other patient, according to established guidelines. In contrast, the frail, compromised elderly and the oldest old require specific measures that target their needs, including with respect to underlying renal conditions. The book considers all those pathologies that have a high prevalence in the general population, explaining advanced treatment concepts and all aspects of assessment. Key clinical points are highlighted, and the text is supplemented with numerous informative figures and tables. The authors are respected experts, for the most part geriatric physicians, and the book is especially addressed to cardiologists, who may lack the described targeted information and treatment tools.
Contents
1. Introduction: The Specificity of Geriatric Cardiology -- 2. Cardiovascular Disease in the Oldest Old: A Geriatric Epidemiology Perspective -- 3. Molecular Mechanisms of Cardiovascular Aging -- 4. Frailty and Heart Disease -- 5. Evidence-Based Versus Evidence-Biased Medicine in Geriatric Cardiology: From Trials to Real World -- 6. Polypharmacy -- 7. Syncope and Unexplained Falls in the Elderly -- 8. Hypertension in the Oldest Old, Beyond Guidelines -- 9. New Challenges in Aortic Stenosis in the Elderly: From Epidemiology to TAVI -- 10. Aging Kidney: Vascular Characteristics and Assessment -- 11. Risk Assessment in Cardiac and Noncardiac Surgery in Older Patients -- 12. Atrial Fibrillation in the Elderly -- 13. Abstract
This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.
Contents
1. Osteoporosis and the nature of fragility fracture: an overview -- 2. Frailty, sarcopenia and falls -- 3. Falls and secondary fracture prevention -- 4. Comprehensive geriatric assessment from a nursing perspective -- 5. Orthogeriatric nursing in the emergency and perioperative in-patient setting -- 6. Mobility, remobilisation, exercise and prevention of the complications of stasis -- 7. Pressure injury prevention and wound management -- 8. Nutrition and hydration -- 9. Nursing the patient with altered cognitive function -- 10. Rehabilitation and discharge -- 11. Family partnerships, palliative care and end of life -- 12. Orthogeriatric nursing.

This book focuses on orthogeriatrics, a topic that has received little attention in the literature to date. As active members of the Fragility Fracture Network (FFN), the editors and most of the authors of this book have gathered all essential expertise on hip fracture management in a single volume. The 14 chapters provide a complete overview of how to approach fractures in elderly patients, starting with the epidemiology of hip fractures and subsequently addressing osteoporosis, surgery, anesthesia, medical management of frailty, and peri-operative complications. Attention is also given to rehabilitation and nursing, as well as to the psychological evaluation of the patient and the caregivers, thus emphasizing the importance of a multidisciplinary approach. Thanks to its clinical and practice-oriented nature, the book will especially be of interest to residents and young geriatricians, as well as orthopaedic surgeons, anesthesiologists and nurses dealing with elderly fracture patients in all parts of the world.
Contents
1. The orthogeriatric approach: progress worldwide -- 2. Epidemiology of hip fracture and social costs -- 3. Osteoporosis in elderly patients -- 4. Frailty, sarcopenia, falls and fractures -- 5. Pre-operative management -- 6. Hip fracture: the choice of surgery -- 7. Orthogeriatric anaesthesia -- 8. Post-operative management -- 9. The nursing role -- 10. Rehabilitation following hip fracture -- 11. Multi-professional team: coordination and communication -- 12. How to implement a fracture liaison service -- 13. This book highlights recent advances in our understanding of sarcopenia and frailty in CKD. The prevalence of sarcopenia and frailty increases substantially as CKD progresses, and experimental studies have demonstrated the pivotal role of accumulated uremic toxin in the development of muscle wasting. Further, sarcopenia and frailty are associated with falls, bone fractures, cognitive impairment, and poor survival prognoses, especially in elderly CKD patients. The syndromes are also related to the risk of end-stage kidney disease. This book provides readers with a deeper understanding of the prevention and management of sarcopenia and frailty in CKD patients to improve their renal and overall health, and suggests directions for future basic and clinical research.
