Abstract
This book examines in detail the topic of sepsis, with a focus on intra-abdominal sepsis. Particular attention is devoted to source control in the management of the infection, antimicrobial therapy and sepsis support, which represent the cornerstones of treating patients with abdominal sepsis. The importance of a multidisciplinary approach is highlighted not only by the instructive and informative sections on the acute manifestations of appendicitis, cholecystitis, and cholangitis, perforations and diverticulitis, Clostridium difficile infection and the role of candida in abdominal sepsis, but also by an extensive discussion of issues including antimicrobial resistance, damage control surgery, principles of antimicrobial therapy, hemodynamic support, adjunctive therapies, and thromboprophylaxis. Abdominal sepsis is the host's systemic inflammatory response to intra-abdominal infections. It is associated with significant morbidity and mortality rates, and represents the second most common cause of sepsis-related mortality at intensive care units. This is the second edition of a well-received book that reflects the state of the art in diagnosis and treatment of acute abdominal disorders in the pregnant patient. It addresses a wide range of conditions, whether associated with or incidental to pregnancy, ranging from very rare to more common ones, such as acute appendicitis and acute cholecystitis. It offers an update on recommendations, guidelines and scenarios to provide fundamental support for all clinicians who might encounter such cases. The book highlights the importance of a rapid diagnosis to avoid serious consequences for both the mother and the fetus. Furthermore, it sheds light on the different imaging modalities of rare pathologies that can occur during pregnancy, helping clinicians and radiologists to better define underlying cases. This new edition has been almost completely rewritten, and includes an additional section focusing on urologic emergencies, preterm labor and intra-abdominal pressure, as well as new figures and tables. It is equally valuable for general and abdominal surgeons, gynecologists and obstetricians, as well as emergency physicians, who may be the first specialists to have clinical contact with this group of patients. Alcohol abuse causes not only liver injury but can harm various organs, resulting in esophageal and colorectal cancers, GERD, pancreatitis, etc. Similar to alcoholic abuse, metabolic syndrome based on obesity and diabetes is also strongly associated with the development of various digestive diseases. Although these diseases may be differentiated by the presence or absence of alcohol intake, the pathologic findings and pathogenesis reveal a number of similarities. This volume covers clinical and basic approaches for esophageal, gastric, hepatic, colorectal and pancreatic diseases associated with alcohol abuse and metabolic syndrome; further, it discusses the roles of microbiota, oxidative stress, and apoptosis, the critical factors causing alcoholic and metabolic digestive diseases.
