Within each category, a comprehensive list of differential diagnoses is discussed. For each disease entity, the reader is updated with new molecular genetic data, biomarkers, and recent applications of immunophenotyping, and how to incorporate the new information in disease diagnosis and classifications is illustrated, including the use of diagnostic algorithms where appropriate. The book employs the revised WHO Classification of Hematopoietic Neoplasms for all disease entities. Diagnosis of Blood and Bone Marrow Disorders will serve as a very useful resource for pathologists, pathologists in training, hematologists and medical technologists who are involved in the clinical work-up of patients with bone marrow and blood neoplasms. This book provides a comprehensive and up-to-date overview of the pathologic features of common benign and malignant hematopoietic disorders in spleen and liver for practicing pathologists, hematopathologists and clinicians. The authors are from large academic centers, affiliated teaching hospitals and central referral clinics and are experienced in the diagnosis of hematopoietic disorders in the spleen and liver. The book consists of 21 chapters, with the first three chapters devoted to normal histologic features, conventional, cytogenetic and molecular studies necessary for the diagnosis of hematopoietic disorders in the spleen and liver. Chapters 4 to 17 cover the primary and secondary mature B- and T/NK cell lymphomas, Hodgkin lymphoma, B and T cell lymphoblastic leukemias, myeloid neoplasms, histiocytic and dendritic neoplasms, and post-transplant disorders including post transplant lymphoproliferative disorders. Chapters 19 to 21 encompass red blood cell disorders, other benign hematologic disorders and infectious/inflammatory disorders that could mimic hematopoietic neoplasms. These chapters are formatted on specific hematopoietic neoplasms to comprise epidemiology, etiology, pathogenesis, morphology, immunophenotyping, molecular genetics, prognosis and brief treatment guidelines. Diagnostic caveats are included in order to have a quick review of the key points in each chapter. Diagnostic Pathology of Hematopoietic Disorders of Spleen and Liver covers most, if not all of the benign and malignant hematopoietic disorders in the spleen and liver and serves as a practical and useful resource for general pathologists and hematopathologists.

The obesity pandemic continues to increase on a world-wide basis with over 70% of the United States population being either overweight or obese. Hematologic malignancies have recently been identified among the obesity associated malignancies spanning the lifespan from childhood to the elderly and include leukemia, myeloma, lymphoma and others. In addition to the potential etiologic association between obesity and hematologic malignancies, the presence of obesity has profound effects on therapy by impacting pharmacokinetics of chemotherapeutic agents, dose, adipocyte metabolism and drug distribution. These may be particularly important in hematopoietic stem cell transplantation. Another important aspect of the association of obesity with hematologic malignancies is the increased incidence of obesity in children who successfully complete therapy for acute lymphoblastic leukemia. This and other observations indicate important relations between the hematopoietic systems and adipose tissue metabolism. Energy Balance in Hematologic Malignancies will provide an important volume in the series on Energy Balance and Cancer and serve as a basis for better understanding etiology, mechanisms, therapeutic implications and experimental approaches. While this book should be of specific interest and utility to all pediatric and adult hematologists and oncologists dealing with hematologic malignancies, it should also provide an important resource for all investigators concerned, from a clinical or research basis, with the impact of obesity and exercise on hematologic malignancies.

Abstract
This book presents a general introduction to and review of HIV-associated hematological malignancies, with a special focus on practical management issues. Each of the relevant malignancies is addressed individually, with an overview of treatment approaches, assessment of evidence regarding their efficacy, and discussion of therapeutic controversies. In addition, careful consideration is paid to issues in molecular and clinical pathology, epidemiological aspects, symptomatology, diagnosis, and risk factors. Separate chapters are devoted to autologous and allogeneic stem cell transplantation and to chemotherapy and interactions with antiretroviral agents. Many of the chapters are written by experts who have been instrumental in shifting the balance for people living with HIV and blood cancers. While two decades ago this diagnosis represented a death sentence, advances in treatment have transformed these cancers into often curable conditions. Nevertheless, optimal treatment of hematological malignancies remains a challenge, particularly in patients with severe immunosuppression. This book will be an invaluable source of information for all practitioners in the fields of clinical hematology and medical oncology and HIV medicine.
