The Seniors Community Health Team is a team of health care providers dedicated to the health, safety, and wellbeing of seniors. They visit seniors in their homes in Lunenburg and Queens Counties. The goal of the program is to help to improve seniors' quality of life, and help them to stay healthy and independent, at home and in the community.
