The importance of a multidisciplinary approach is highlighted not only by the instructive and informative sections on the acute manifestations of appendicitis, cholecystitis, and cholangitis, perforations and diverticulitis, Clostridium difficile infection and the role of candida in abdominal sepsis, but also by an extensive discussion of issues including antimicrobial resistance, damage control surgery, principles of antimicrobial therapy, hemodynamic support, adjunctive therapies, and thromboprophylaxis. Abdominal sepsis is the host's systemic inflammatory response to intra-abdominal infections. It is associated with significant morbidity and mortality rates, and represents the second most common cause of sepsis-related mortality at intensive care units. Antimicrobial resistance is a major threat to the well-being of patients and health systems the world over. In fragile health systems so challenged, on a day-today basis, by the overwhelming burden of both infectious and non-communicable disease, it is easy to overlook the impacts of AMR. The Maternal Sepsis Intervention, focusing on a primary cause of maternal death in Uganda, demonstrates the systemic nature of AMR and the gains that can be made through improved Infection Prevention Control and direct engagement of laboratory testing in antibiotic prescribing. Louise Ackers holds a Chair in Global Social Justice at the University of Salford, UK, and has recently been appointed as a consultant for the World Health Organization. Gavin Ackers-Johnson is Researcher at the University of Salford, UK. Joanne Welsh is Researcher at the University of Salford, UK. - Bacterial Pathogens and their Virulence Factors contains a detailed description of 32 major bacterial pathogens that affect human health and their associated virulence determinants. Chapter 1 gives an overview of the different types and classes of general virulence factors involved in host cell adherence and invasion, dissemination within the host, host cell damage, and evasion of host defense systems, as well as mechanisms by which these virulence factors are regulated. Chapters 2 through 33 give concise descriptions of the disease states associated with the 32 bacterial genera and their major pathogenic species, along with an in-depth description of the individual virulence factors that have been found to be functionally involved in pathogenicity. By a combination of stochastic modeling and economic geography, this book proposes two key causes together explain the explosive spread of the worst of the vector-borne outbreaks. Ecosystems in which such pathogens are largely controlled by environmental stochasticity are being drastically streamlined by both agribusiness-led deforestation and deficits in public health and environmental sanitation. Consequently, a subset of infections that once burned out relatively quickly in local forests are now propagating across susceptible human populations whose vulnerability to infection is often exacerbated in structurally adjusted cities. The resulting outbreaks are characterized by greater global extent, duration, and momentum. As infectious diseases in an age of nation states and global health programs cannot, as much of the present modeling literature presumes, be described by interacting populations of host, vector, and pathogen alone, a series of control theory models is also introduced here. These models, useful to researchers and health officials alike, explicitly address interactions between government ministries and the pathogens they aim to control.
