Abstract
Current economic, demographic, and environmental shifts are presenting major challenges to health care systems around the world. In response, decentralization--the transfer of control from central to local authorities--is emerging as a successful means of meeting these challenges and reducing inequities of care. But as with health care itself, one size does not fit all, and care systems must be responsive to global reality as well as local demand. Decentralizing Health Services explores a variety of applications of decentralization to health care delivery in both the developing and developed worlds. Outfitted with principles, blueprints, and examples, this ambitious text clearly sets out the potential role of decentralized care as a major player in public health. Its models of service delivery illustrate care that is effective, inclusive, flexible, and in tune with the current era of preventive and evidence-based healthcare. Contributors point out opportunities, caveats, and controversies as they: Clarify the relationships among decentralization, politics, and policy Differentiate between political, fiscal, and administrative decentralization in health care systems. Consider public/private partnerships in health systems. Explain how the effects of decentralization can be evaluated. Present the newest data on the health outcomes of decentralization. Explore some challenges and global issues of health systems in the 21st century. And each chapter features learning goals, discussion questions, activities, and recommendations for further reading. Heralding changes poised to revolutionize care, Decentralizing Health Services will broaden the horizons of researchers and administrators in health services, health economics, and health policy.

Abstract
Biobank research and genomic information are changing the way we look at health and medicine. Genomics challenges our values and has always been controversial and difficult to regulate. In the future lies the promise of tailored medical treatments and pharmacogenomics but the borders between medical research and clinical practice are becoming blurred. We see sequencing platforms for research that can have diagnostic value for patients. Clinical applications and research have been kept separate, but the blurring lines challenges existing regulations and ethical frameworks. Then how do we regulate it? This book contains an overview of the existing regulatory landscape for biobank research in the Western world and some critical chapters to show how regulations and ethical frameworks are developed and work. How should international sharing work? How design an ethical informed consent? An underlying critique: the regulatory systems are becoming increasingly complex and opaque. The international community is building systems that should respond to that. According to the authors in fact, it is time to turn the ship around. Biobank researchers have a moral responsibility to look at and assess their work in relation to the bigger picture: the shared norms and values of current society. Research ethics shouldn't only be a matter of bioethicists writing guidelines that professionals have to follow. Ethics should be practiced through discourse and regulatory frameworks need to be part of that public discourse. Ethics review should be then not merely application of bureaucracy and a burden for researchers but an arena where researchers discuss their projects, receive advice and practice their ethics skills.

Abstract
This open access book provides a set of conceptual, empirical, and comparative chapters that apply a public policy perspective to investigate the political and institutional factors driving the use of evidence to inform health policy in low, middle, and high income settings. The work presents key findings from the Getting Research Into Policy (GRIP-Health) project: a five year, six country, programme of work supported by the European Research Council. The chapters further our understanding of evidence utilisation in health policymaking through the application of theories and methods from the policy sciences. They present new insights into the roles and importance of factors such as issue contestation, institutional arrangements, logics of appropriateness, and donor influence to explore individual cases and comparative experiences in the use of evidence to inform health policy. Justin Parkhurst is Associate Professor at the London School of Economics and Political Science (the LSE)’s Department of Health Policy, UK. He has conducted research on a range of global health policy issues and on the politics of evidence. He served as the Principal Investigator of the GRIP-Health programme of work. Benjamin Hawkins is Associate Professor at the Department of Global Health and Development, London School of Hygiene and Tropical Medicine, UK. His research focuses on the role of research evidence and corporate actors in health policy making. In addition, he works on European integration, multi-level governance international trade and political economy approaches to health policy. Stefanie Ettelt is Associate Professor at the Department of Health Services Research and Policy, London School of Hygiene and Tropical Medicine, UK. Her work examines the tensions between structure and agency in explaining the influence of evidence and research on policy-making and health system governance, particularly from a comparative perspective.

Abstract
Global Health and Volunteering: A Guide for Healthcare Professionals is designed to educate volunteers to be effective partners in delivering medical services locally and globally. Healthcare professionals are increasingly interested in global health and volunteering in areas of acute need. The biggest challenge to health in many locales is the inability to access the health care system. When people do connect with medical services, medications and surgical opportunities for chronic disease (i.e. glaucoma, diabetes, or hypertension) are often not affordable or cannot be sustained for a long period of time. The contributions in this book focus on a respectful dialog with local people and a willingness to learn from new experiences on the part of the volunteer. Skills transfer from visiting personnel to local providers is featured as a means to enhance healthcare sustainability. An appreciation of differing cultures, an understanding of the local economic conditions and challenges, and strategies for collaborating with the existing medical establishment are foundations of successful volunteer experiences as highlighted in this book. Dimensions of global health such as professionalism, religious beliefs, ethical dilemmas, traditional medicine, and alternative strategies for service are addressed by experts. Written and edited by leaders in the field, many of whom have more than two decades of experience volunteering abroad, Global Health and Volunteering: A Guide for Healthcare Professionals imparts lessons learned to help the reader avoid initial mistakes, while making the global health commitment stronger.

Abstract
This book discusses globalization and its impact on human health. The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. These changes produce complex and constantly varying interactions between the biosphere, economy, climate and human health, forcing us to approach future global health trends from a new perspective. Preventive actions to improve health, especially in low-income countries, are essential if our future is going to be a sustainable one. After a period of undeniable improvement in the health of the world's population, this improvement is likely to slow down and we will experience--at least locally--crises of the same magnitude as have been observed in financial markets since 2009. There is instability in health systems, which will worsen if preventive and buffering mechanisms do not take on a central role. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of health as a common good. An entirely competition-driven economy cannot--by its very nature--address global challenges that require full international cooperation. A communal global leadership is called for. Paolo Vineis is Chair of Environmental Epidemiology at Imperial College. His current research activities focus on examining biomarkers of disease risk as well as studying the effects of climate change on non-communicable diseases. "From morality to molecules, environment to equity, climate change to cancer, and politics to pathology, this is a wonderful tour of global health: consistently presented in a clear, readable format. Really, an important contribution." Professor Sir Michael Marmot Director, Institute of Health Equity University College London Author of "The Health Gap". "This book is a salutary and soundly argued reminder that the "common good" is not simply what remains after individuals and groups have appropriated the majority of societal resources: it is in fact the foundation on which any society rests and without which it collapses." Rodolfo Saracci Senior Visiting Scientist International Agency for Research on Cancer, Lyon, France.

Abstract
A timely work describing how localized hospital-based health technology assessment (HB-HTA) complements general, "arms-length" HTA agency efforts, and what has been the collective global impact of HB-HTA across the globe. While HB-HTA has gained significant momentum over the past few years, expertise in the field, and information on the operation and organization of HB-HTA, has been scattered. This book serves to bring this information together to inform those who are currently working in the field of HTA at the hospital, regional, national or global level. In addition, this book is intended for decision-makers and policy-makers with a stake in determining the uptake and decommissioning of new and established technologies in the hospital setting. HTA has traditionally been performed at the National/Regional level by HTA Agencies, typically linked to governments. Yet hospitals are the main entry door for most health technologies (HTs). Hospital decision-makers must undertake multiple high stakes investment and disinvestment decisions annually for innovative HTs, usually without adequate information. Despite the existence of arms-length HTA Agencies, inadequate information is available to hospital decision-makers either because relevant HTA reports are not yet released at the time of entry of new technologies to the field, or because even when the report exists, the information contained is insufficient to clarify the contextualized informational needs of hospital decision makers. Therefore, there has recently been a rising trend toward hospital-based HTA units and programs. These units/programs complement the work of National/Regional HTA Agencies by providing the key and relevant evidence needed by hospital decision makers in their specific hospital context, and within required decision-making timelines. The emergence of HB-HTA is creating a comprehensive HTA ecosystem across health care levels, which creates better bridges for knowledge translation through relevance and timeliness.

Abstract
This book discusses the latest findings on the pathogenesis and treatment of IgA nephropathy. It particularly focuses on recently recognized initiation and progression factors and the varying treatment strategies in different regions, such as Asia, Europe, and the United States. More than 40 years have passed since Dr. Jean Berger first described primary IgA nephropathy ("Nephropathy with mesangial IgA-IgG deposits") as a new disease entity. Immunohistopathologically, IgA nephropathy is characterized by the granular deposition of IgA (IgA1) and C3 in the glomerular mesangial areas with mesangial cell proliferation and the expansion of mesangial matrices. It is clear that IgA nephropathy is one of the most common types of chronic glomerulonephritis in the world. This disease may lead to end-stage kidney disease, with its enormous economic impact on healthcare everywhere. Efforts by many investigators around the world have gradually clarified various aspects of the pathogenesis and treatment of IgA nephropathy. However, there are many controversial strategies for the treatment of patients with IgA nephropathy throughout the world, as there are several limitations for treatment in each country. This volume provides nephrologists everywhere with an overview and comparison of both global and regional findings in basic and clinical fields in IgA nephropathy. It covers genetic variation, aberrant IgA1 production, and classification etiology, guidelines, and treatment goals, with all chapters written by top international researchers.

Abstract
Written by experienced authors, this book offers expert personal views on what the current problems in pharmacovigilance are and how they should be solved. This book stems from thoughts and ideas discussed in a series of meetings of the International Society of Pharmacovigilance (ISoP), where concerns were raised that the current pharmacovigilance system is not delivering optimally to improve therapeutics in clinical practice. Pharmacovigilance of the future must be an active and integral part of health care delivery, and focus more on science and practices that support health professionals and patients in day-to-day care situations. To achieve this, a dynamic and sustainable development of vigilance must take precedence over the current excessive preoccupations with data processing and regulations; all aspects of medicines use and their effects need to be considered; and all stakeholders must be involved and engaged in an open and constructive debate. The work is essential reading for anyone who has an interest in safer use of medicines. It is intended to be equally challenging and rewarding, and sets out to stimulate a continuous debate on how pharmacovigilance can better meet the needs of health professionals and patients to achieve the aim of wise therapeutic decision making.

Psychosocial problems appear within a medical context worldwide, and are a major burden to health. Psychosomatic Medicine: An InternationalPrimer for the Primary Care Setting takes a uniquely global approach in laying the foundations of biopsychosocial basic care (such as recognizing psychosocial and psychosomatic problems, basic counseling, and collaboration with mental health specialists) and provides relevant information about the most common mental and psychosomatic problems and disorders. The scope of the book is interculturalit addresses global cultures, subcultures living in a single country, and strengthening the care given by physicians working abroad. This clinically useful book outlines best practices for diagnosing the most common biopsychosocialproblems and mastering the most common communication challenges (e.g. doctor-patient conversation, breaking bad news, dealing with difficult patients, family and health systems communication and collaboration). Every chapter integrates basic theoretical background and practical skills and includes trans-culturally sensitive material, important for work with patients from different cultures. Abstract
This volume explores the unique challenges midwifery graduates face as they move into practice. It identifies the similarities and differences in midwifery education, regulation, and clinical practice faced by graduate midwives in all continents, examining the various support systems available for graduate midwives in many countries, and identifying the common strategies (formal and informal) and approaches that have proved to be effective in supporting midwifery graduates. The book volume brings together the experiences of new midwives starting out in registered practice, to share the challenges and triumphs during their transition to confident practitioners. It identifies, explains and details both established and innovative new mechanisms in place to support new midwives in each country, and examines the effects the experiences of transitioning to practice may have on future professional practice, resilience and sustainability. Lack of support during the new-graduate transition to practice has been associated with early attrition from the midwifery profession. Stress, disillusion, and horizontal violence have been identified as factors that influence midwifery attrition rates. Exploration of the various support mechanisms currently available in different countries may stimulate the sharing of best practices in providing new midwives with transition to practice programmes and generate further research. Each chapter is harmonized to facilitate the comparison between countries, and the maternity services context is explained using each country’s specific legislation, regulation and registration of midwives. The preparation of midwifery students for qualified practice is outlined to explain how midwifery students are trained and socialized into the profession, mentored in their placements and then transitioned to registered midwife status. This book appeals to midwives, managers, educators, and newly graduated interested in international midwifery practice.
