Abstract
This book describes the approach to anesthetic and perioperative management in different categories of high-risk patient scheduled to undergo elective noncardiac surgery. Individual sections focus on patients with conditions entailing cardiac and hemodynamic risks, respiratory risks, renal and metabolic risks, neurological risks, and other forms of risk. For each condition, up-to-date guidance is provided on risk evaluation, risk stratification, and intraoperative and postoperative anesthetic management. This is the first time that such different clinical situations have been gathered together in a single textbook of anesthesiology. All of the authors are international experts with extensive clinical experience. The aim is to provide trainee anesthesiologists with clear guidance that will prove invaluable when managing a wide range of patients with serious illnesses or conditions that pose a significant anesthetic risk. The book will also serve as a valuable reference for more experienced anesthesiologists and intensivists.

Abstract
This text provides a background of scientific evidence to understand the complications that occur after bariatric surgery for all providers responsible for care after surgical intervention. Written by experts and based on current peer reviewed literature, the text provides a focused approach to the identification and treatment of bariatric surgery complications. To establish a context for providers, the initial section concentrates on presenting the current procedures as well as risks and expected benefits of each with an emphasis on mechanism of action. It examines the issue of weight regain from the aspect of heterogeneity in order to present the perspective that every procedure has risk of weight regain. The text provides guidance to those front line providers who manage acute emergencies and chronic long term problems. State of the art management of complications like leaks, bleeding, ulcers, blood clots and pneumonia are discussed for all procedures in addition to the management of rapid remission of obesity related disease like Type 2 Diabetes and coagulopathies (Leiden Factor V, Protein C and S) that occur commonly in patients with obesity. Each chapter features a review of the current literature in an easy to reference table format and where appropriate an algorithm to focus the reader on the process of care for any given presenting sign or symptom. Bariatric Surgery Complications will be of great value to fellows in minimally invasive surgery, general surgeons, emergency room physicians, gastroenterologists, primary care physicians, medical students and residents in surgery rotations, integrated health personnel.
Contents

Abstract
This book is an up-to-date, well-referenced practical resource that offers detailed guidance on the avoidance and management of complications in patients treated for cerebrovascular and spinal vascular disease. All complication avoidance and management techniques currently available to the endovascular/cerebrovascular surgeon are reviewed by pioneers and leaders in the field to provide the clinician with an advanced single point of reference on the subject. The book is divided into four sections. It opens by discussing general issues, such as definition of complications, medicolegal aspects, the role of resident training, and checklists. The subsequent three sections address the avoidance and management of complications when performing surgical, endovascular, and radiosurgical procedures, covering the full range of indications and potential adverse events. All chapters have a standardized format, simplifying the search for information on a specific disease process. Numerous intraoperative images are included, and, when appropriate, algorithms for the avoidance, early recognition, and management of complications are presented. Each chapter concludes with a checklist of preparatory steps and "emergency procedures" that each member of the team must perform in order to ensure the best possible outcomes.

Abstract
The book presents more than 60 real-life cases which together memorably and succinctly convey the depth and breadth of clinical anesthesiology. Each chapter includes a case summary, questions, lessons learned, and selected references. Tables and distinctive visual synopses of key teaching points enhance many chapters. The cases have been selected by Dr. Benumof from the Morbidity and Mortality (M & M) conferences of the Department of Anesthesiology, University of California, San Diego, which he has moderated the last several years, and residents and junior faculty have crafted them into the chapters of this book. Structured in a novel way, the UCSD Anesthesiology M & Ms maximize teaching and learning, and these cases bring that experience right to the reader's finger tips. Case coverage of respiration- and circulation-related problems, obstetrics, neurology, pain and regional anesthesia, pediatrics, outpatient surgery, and special topics; Resource for anesthesiology and critical care medicine trainees; Review tool for board certification or recertification; Fun reading; Valuable lessons.

Abstract
This practical text presents the most up-to-date information on the evaluation and management of all aspects of complex and revision shoulder arthroplasty, divided into three thematic section. Chapters in part I focus on the diagnosis and management of complex primary shoulder arthroplasty, including evaluation of humeral and glenoid bone deformity, computer-assisted surgical planning, stemless arthroplasty, anatomic and reverse TSA for severe glenoid erosion, post-traumatic sequalae and deltoid deficiency. The next section describes the diagnosis and management of failed replacements, including hemiarthroplasty, anatomic and reverse total shoulder arthroplasty. These chapters will focus on the initial evaluation and management, including imaging, laboratory work-up and the role of biopsies, covering the diagnosis and revision of a failed hemiarthroplasty; diagnosis and revision of a failed total shoulder arthroplasty; diagnosis and management of the failed reverse shoulder arthroplasty; management of the unstable TSA or RSA; diagnosis and management of the infected arthroplasty; treatment of periprosthetic fractures; salvage options (resection and arthrodesis) for failed arthroplasty. Part III presents specific surgical techniques utilized during revision shoulder arthroplasty, including surgical exposure, removal techniques for failed humeral components, revision of glenoid and humeral defects with bone grafts and augments, and the role of arthroscopic procedures. Utilizing the best clinical evidence, Complex and Revision Shoulder Arthroplasty is an excellent resource for orthopedic and shoulder surgeons, residents and fellows, as well as sports medicine specialists and related professionals.

Abstract
Complexities in Colorectal Surgery: Decision-Making and Management provides a unique, modern, practical guide that covers the strategic evaluation, specific approaches, and detailed management techniques utilized by expert colorectal surgeons caring for patients with complex problems whether they result from underlying colorectal disease or from complications arising from previous surgical therapy. The text is formatted as both a "how-to" manual as well as an algorithm-based guide allowing the reader to understand the thought process behind the proposed treatment strategies. By making use of evidence-based recommendations, each chapter will include not only diagnostic and therapeutic guidelines, but also a narrative by the author on his/her operative technical details and perioperative "tips and tricks" that they utilize in the management of these complex surgical challenges. This test includes chapters on the assessment of risk and nutritional intervention methods utilized to minimize perioperative complications. In addition, sections on medical and surgical therapies for abdominal, pelvic and anorectal disease are incorporated. Moreover, the technical challenges of managing complications resulting from the original or subsequent operations is addressed. The underlying focus throughout the text is to provide pragmatic and understandable solutions that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems. The text also goes beyond the technical aspects of colorectal surgery and includes special sections highlighting the essence of a surgeon. These include chapters describing the importance of the first encounter, building patient rapport, and demonstrating confidence and competence while showing humility and avoiding arrogance. Additionally, aspects involving the medical-legal, ethical, and economic challenges confronting surgeons are covered.
