Abstract
This book describes the diagnoses; staging and management of patients with colorectal liver metastases initially considered unresectable and portrays the different strategies to increase resectability along with their tactics and tricks. Colorectal carcinoma is the third most commonly diagnosed cancer in the world and according to recent cancer statistics around 1.23 million patients are diagnosed each year. Of these patients, approximately 50% will develop liver metastases during the course of their disease and around 15-25% are found to have stage IV disease at diagnosis. Liver resection has been recognized as the treatment of choice for these patients, offering overall 5-year survival rates of up to 50-60% and the only hope for cure. However, at diagnosis only 10-20% of these patients are possibly amenable to surgical resection with curative intent. The possibility to achieve an R0 resection is many times limited by the amount and quality of the future liver remnant (FLR), being posthepatectomy liver failure (PHLF) the most feared and severe complication after major liver resections. With the years, diverse strategies have been developed with the intention to increase resectability by increasing the future liver remnant and/or reducing tumor size, e.g. ALPPS. Along with these techniques, associated surgeries are developed including multivisceral resections, which broadens even more the resectability for patients.

This book offers a complete overview on noncolorectal nonneuroendocrine (NCRNNE) liver metastases and describes in detail the currently available therapies. Each chapter focuses on the treatment of metastases from a particular primary malignancy and also provides valuable information on incidence, natural history, and diagnosis. NCRNNE metastases to the liver are rare entities compared with colorectal and neuroendocrine metastases, for which the treatments are well codified. While more publications have appeared on the topic in recent years, an in-depth study has to date been lacking. Furthermore, most published series are insufficiently comparable as they comprise patients with NCRNNE hepatic metastases from a variety of primary malignancies and consequently overlook differences in tumor behavior, frequency of isolated hepatic metastases, response to neoadjuvant or adjuvant therapy, and interval between diagnosis of the primary tumor and the liver metastases. This book, with its more specific approach, will serve not only as an up-to-date guide to diagnosis and treatment but also as a reference on which to base future studies.

Abstract

RFA has emerged as a new treatment modality and become the main modality of locoregional therapy. Extensive clinical research indicates that RFA is as effective as surgical resection for sHCC, and it has the advantage of being less invasive. However, the outcomes after RFA are largely dependent on the operators' experience--known as the "learning curve". This book presents the characteristics of sHCC and discusses why sHCC is the best candidate for RFA. Then it introduces all the commercially available RFA systems, and their working principles, advantages, disadvantages and so on. It goes on to demonstrate how to perform RFA under the guidance of ultrasound, CT, laparoscopy, or during open operation. Finally, it discusses the radiologic assessment and follow-up after RFA, as well as adjuvant therapies and clinical trials on RFA. The authors are experts from the fields of pathology, radiology, surgery, and gastroenterology, as well as manufacturers. With this book, readers gain have a clear idea of when and how to do RFA. It aims to standardize and generalize the procedure of RFA, which will be very helpful in improving the outcome of RFA for sHCC.
