Members of the Multi-Organ Transplant Program Team (Transplant Team) are also available to talk with you about your concerns. Everyone is different, so you may not have or develop all of the conditions that you read about in this guide. Topics include: in the hospital, monitoring, possible complications, planning for discharge, follow-up care, taking care of yourself, and the self-medication program. This guide also includes a sample medication record. Medication information sheets are provided at the end of the guide if you have specific questions about your medications. Information about common tests (lab tests) is given. Important phone numbers are listed. The French version of this pamphlet 1124, "Après une greffe du foie", is also available.
