Abstract

The Atlas of Osteoarthritis concludes with the latest treatment updates including both nonpharmacological and pharmacological treatments, as well as surgical recommendations for patients with the disease. Osteoarthritis is the most common form of joint disease causing joint pain, stiffness, and physical disability among adults. It is an important issue for both the individual and society with its impact on public health continuing to grow as a result of the aging population, the rising prevalence of obesity, and the lack of definitive treatments to prevent or halt the progress of the disease.

Detailed analyses are given of the role of the synovium, of the molecular mechanisms that lead to degradation of the cartilage matrix, of the hypertrophy of the cartilage cell, of the anabolic and catabolic roles of cytokines, may lead to novel approaches to clinical treatment, utilizing anabolic mediators or molecules that target steps in the disease process. Also discussed are animal models and how mechano-responsiveness is compromised by mechanical injury. Orthopedics and rheumatology have become close conceptually, as advances in bone and joint biology have enabled bench and translational scientists, as well as practitioners, to approach clinical problems comprehensively. Because bone plays a role in initiating osteoarthritis, therapeutic approaches focusing on bone tissue are included in the discussion of novel treatments. The general topic of osteoarthritis is therefore a timely subject for a series on bone biology. This book, intended for clinicians, researchers and students, provides information that will orient the novice and update the specialist. No other book treats the relationship of bone to osteoarthritis in similar fashion or provides a comparable underpinning of joint pathophysiology.

Abstract
Osteoarthritis (OA) is among the top 10 of most disabling diseases in the Western world. It is the major cause of pain and disability among the elderly. This book provides a contextual review of recent research on neuromuscular factors and behavioral risk factors for functional decline in OA, with a special emphasis on explanatory mechanisms. In addition, the book discusses innovative approaches to exercise and physical activity in OA, derived from research on behavioral and neuromuscular risk factors for functional decline in OA. Recent research has shown that neuromuscular factors (such as muscle strength, joint laxity) and behavioral factors (such as avoidance of activity, depressed mood) predict pain and disability in OA. Furthermore, exercise and physical activity are among the dominant interventions aiming at reducing pain and disability, and innovative interventions targeting neuromuscular and behavioral interventions have been recently developed. This research has been published as separate papers, with the result that the field is in need of an integrative contextual review that puts the research into theoretical perspective.

Abstract
This book is a rich source of practical guidance on the management of relatively young patients with osteoarthritis of the knee that will assist physicians and allied health professionals in enabling patients to remain active and sustain their quality of life. It provides up-to-date knowledge on the available treatment options, describes the evidence base for each option, and identifies the appropriate timing and indications. The book opens by considering the basic science behind osteoarthritis, which is critical to management. The many nonsurgical treatment modalities are then discussed, with particular focus on the importance of a multidisciplinary approach. Subsequent chapters address the role of surgical management, covering both techniques that attempt to preserve and possibly restore the native knee joint, such as meniscal and chondral surgery, arthroscopic debridement, and osteotomy for realignment of the joint, and those that involve joint replacement. The arthroplasty component of the text encompasses all areas of prosthetic resurfacing, including localized resurfacing, unicompartmental replacement, and total knee replacement. The book has been produced in cooperation with ISAKOS and the authors constitute an international faculty who provide a truly global perspective on the subject.

This book further discusses the potential of regenerative therapies and recent advances in OA Personalized Medicine, and how collection of OA patients' phenotypic, genetic and proteomic data is able to direct treatment strategies through Bio-Informatics. Dr. Mohit Kapoor is an Associate Professor in the Department of Surgery and the Department of Laboratory Medicine and Pathobiology at the University of Toronto, Canada. He heads the cartilage biology research group within the Arthritis Program at the Toronto Western Research Institute, University Health Network, Toronto. His research program is directed towards understanding the complex pathophysiology of OA and identifying new therapeutic strategies to identify and counteract the destruction of the articular cartilage during OA. Specifically, his research is focused towards (a) understanding the mechanisms that are essential for maintaining articular cartilage homeostasis and mechanisms that trigger cell death and articular cartilage damage, and (b) identifying new OA biomarkers. Dr. Kapoor has published over 50 research articles, reviews and book chapters and is a recipient of several prestigious research awards. Dr. Nizar Mahomed is head of the Division of Orthopaedic Surgery at the University Health Network and Professor in the Department of Surgery at the University of Toronto, Canada. He is Nicki and Bryce Douglas Chair and Smith & Nephew Chair in Orthopaedic Surgery. Clinically he focuses on complex primary and revision hip and knee replacement surgery. He has over 100 publications and book chapters. His research interests are in the areas of health systems research focusing on models of care for orthopaedic conditions and translational research looking at regenerative options for OA. Dr. Mahomed has received several awards for his research and is the recipient of the first CIHR-CMAJ Top Canadian Achievements in Health Research Award. As Medical Director of the Arthritis Program at University Health Network, he leads the largest integrated Arthritis Program in Canada.

Abstract
This book describes the anatomy and biomechanics of the trapeziometacarpal joint and explains the pathogenesis and treatment of trapeziometacarpal joint osteoarthritis, also known as rhizarthrosis. The discussion of treatment sets out both conservative and surgical approaches, clearly explaining the indications for the various options, as well as their advantages and disadvantages. The trapeziometacarpal joint is a phylogenetically recent articulation that permits the pinching movements of the index finger and thumb so important in daily activities. Degenerative disease involving the trapeziometacarpal joint is an important disabling condition that affects predominantly females over 50 years old. Although a number of treatments are now available, there is no single gold standard. Conservative treatments can control pain yet are unable to halt progression of the articular aging, while none of the surgical solutions employed when conservative treatments prove insufficient can be considered perfect. For example, use of a spacer can restore strength but does not always completely alleviate pain while arthroplasty eradicates pain within a few weeks but cannot restore strength. In thoroughly reviewing the available treatments, this book will enable the practitioner to select the best option for the individual patient.
