This book serves as a guide to pathologists to successfully avoid errors and deliver the best diagnosis possible with all relevant information needed to manage patients. The introductory section includes general principles and ideas that are necessary to understand the context of error reduction. In addition to general principles of error reduction and legal and regulatory responsibilities, a chapter on regulatory affairs and payment systems which increasingly may be impacted by error reduction and improvement activities was added. This later chapter is particularly important in view of the implementation of various value-based payment programs, such as the Medicare Merit-Based Incentive Payment System that became law in 2015. The remainder of the book is organized in a similar manor to the 1st edition with chapters devoted to all aspects of the test cycle, including pre-analytic, analytic and post-analytic. The 2nd Edition of Error Reduction and Prevention in Surgical Pathology serves as an essential guide to a successfully managed laboratory and contains all relevant information needed to manage specimens and deliver the best diagnosis.

Making an accurate diagnosis from a limited sample can be quite challenging yet crucial to patient care. The handbook, using the most current and evidence-based resources, will: 1) emphasize a triple test (clinical, radiologic, and cytologic/histologic correlation); 2) stress the importance of cellblock preparation/small tissue core biopsy in modern cytopathology; 3) include over 100 diagnostic immunostaining panels/tables to address the utility and importance of ancillary studies; 4) incorporate relevant FISH and molecular tests into each chapter; and 5) summarize the pearls and pitfalls of FNA/small tissue biopsies at the beginning of each chapter. The book also includes highly practical presentations of typical case scenarios seen in an anatomic pathology laboratory. It details the gross description, histological classification, tumor differentiation, extent of local tumor spread, involvement of lymphovascular channels, lymph nodes and excision margins of the common carcinomas and also summarises non-carcinomatous malignancies. The 7th edition TNM and WHO classifications of cancers are incorporated, with comments on any associated pathology, diagnostic clues and prognostic criteria. Staging information is supplemented visually by line diagrams. Each chapter has a more formal introductory paragraph than in the previous edition, giving epidemiological, clinical, investigative and treatment summary details, and chapter references are updated; specimen type notes new clinical procedures. Other pathology includes updated immunophenotypic expression with use of new antibodies and molecular techniques. The impact of these ancillary investigations on diagnosis, and as biomarkers of prognosis and prediction of response to treatment is summarised. The effect of adjuvant treatments on cancers is also noted. Experience based clues are given throughout as aids to tumor typing, grading, staging and gauging prognosis and response to treatment. The key information in this book equips the pathologist to produce high quality, clinically appropriate histopathology reports, and to participate in contemporary multidisciplinary team management of patients with surgical cancer.
