Abstract
People age 65 and older are the fastest growing segment of the U.S. population. In the 2010 census 16% of the population, 50 million people, were age 65 and older. That number is projected to increase to 66 million by the year 2050. Life expectancy has also increased, with recent CDC reports indicating life expectancy at 77.9 years. Age-adjusted death rates have decreased significantly with the largest changes occurring in older patients. Despite these trends, the 10 leading causes of death include several pulmonary etiologies including lung cancer, chronic respiratory diseases, influenza and pneumonia. Aging and Lung Disease: A Clinical Guide is devoted to understanding the impact of respiratory diseases in older patients. It includes reviews of physiology of the aging lung, allergy and immunology of the aging, as well as sleep changes over the life cycle. There are also comprehensive reviews on specific disease topics including chronic obstructive lung disease, lung cancer, atypical mycobacteria, interstitial lung disease, pulmonary hypertension, pulmonary embolism, obstructive sleep apnea, and sleep disorders in older patients. Two chapters focus on unique issues in older patients; HIV and lung transplant. Included also are important chapters on assessing functional and cognitive status and end-of-life issues in older patients with lung disease. In addition to outlining the current state of knowledge, each chapter focuses on special considerations when caring for older patients. Of particular interest to pulmonologists, internists, and gerontologists, other readers, such as pulmonary and geriatric nurse practitioners, as well as clinical researchers interested in both pulmonary and aging issues, will find Aging and Lung Disease: A Clinical Guide to be a vital resource for improving their care of older patients with lung disorders.
Contents
Physiologic changes -- Physiologic changes in the aging lung -- Allergy and immunology of the aging lung -- Physiologic changes in sleep with aging -- Lung diseases in older patients: epidemiology, presentation, diagnosis, treatment -- Chronic obstructive pulmonary disease in older patients -- Lung cancer in older patients -- Pulmonary hypertension in older patients -- Pulmonary embolism in older patients -- Obstructive sleep apnea in older patients -- Sleep disorders in older patients -- Nontuberculous mycobacterial infections in older patients -- Interstitial lung disease in older patients -- Lung disease in older patients with HIV -- Special considerations in lung transplantation in older patients -- Assessment of functional and cognitive status in older patients with lung disease -- Addressing end-of-life issues in older patients with lung disease.

Abstract
From acute respiratory diseases to chronic lung conditions, pulmonology is a vital area of clinical medicine. Due to a wide array of respiratory diseases, clinical presentation of pulmonary conditions can be complex and oftentimes challenging. In addition to recognizing common characteristics of lung diseases, identifying unusual clinical presentation of pulmonary conditions remains an important educational experience. This concise and easy-to-read title provides that experience in practical context, using a case-based system of learning where a unique case is presented and discussed and questions are provided to challenge the learner and reinforce the transmission of knowledge. Challenging Cases in Pulmonology is not an exhaustive review but rather focuses on common and important problems, with emphasis on evaluation and treatment. The book offers 16 chapters consisting of two or more cases followed by five to ten multiple choice questions. A valuable resource for pulmonologists, allergists, primary care physicians, internal medicine, fellows in allergy and pulmonology, and residents in training, this practical title will also be of interest to nurses, physician assistants, and medical students studying pulmonology. Challenging Cases in Pulmonology complements two previously published case-based titles edited by Dr. Mahmoudi: Challenging Cases in Allergy and Immunology (Humana Press, 2009) and Challenging Cases in Allergic and Immunologic Diseases of the Skin (Springer, 2010)
Contents
Allergic Asthma -- Allergic Bronchopulmonary Aspergillosis -- Hypersensitivity pneumonitis -- Pulmonary Eosinophilic Conundrums -- Occupational Asthma To Low and High Molecular Weight Agents -- Malignant Pleural Mesothelioma -- Pneumonia -- Tuberculosis in Immunocompromised Hosts -- Asthma -- Chronic Obstructive Pulmonary Diseases Conundrums -- Pulmonary Hypertension -- Pulmonary Embolism -- Pulmonary Vasculitis Conundrums -- Diffuse Parenchymal Lung Diseases -- Pulmonary manifestations of Systemic Diseases -- Sarcoidosis -- Cystic fibrosis.

Abstract
Challenging Cases in Rheumatology and Diseases of the Immune System is the latest title in a growing collection of thought-provoking case-based titles from Massoud Mahmoudi, D.O., Ph.D. Like his three preceding titles, Challenging Cases in Allergy and Immunology (2009), Challenging Cases in Allergic and Immunologicá Diseases of the Skin (2010), and Challenging Cases in Pulmonology (2011), this easy-to-read title presents the topic in a challenging and enjoyable case-based format. Developed by 30 distinguished contributors, the book consists of six parts and 16 chapters, with each chapter presenting two cases. The style of this title follows the previous books: each topic begins with an abstract, followed by a case presentation, working diagnosis, data, final diagnosis, and discussion. In addition, to enhance a review of the subject and stimulate critical thinking, there are 5 to 10 multiple choice questions and answers in each chapter. Challenging Cases in Rheumatology and Diseases of the Immune System is an indispensable resource for all clinicians who care for patients with rheumatic and immunologic disorders.

Abstract
Chronic Obstructive Pulmonary Disease (COPD) is a major cause of morbidity and mortality worldwide. By 2020, COPD is expected to be the fifth leading cause of disability and the third leading cause of death. COPD is associated with significant co-morbid illnesses including cardiovascular disease, pulmonary vascular disease, osteoporosis, depression and thoracic malignancies. In addition, COPD patients often have systemic manifestations of their disease, including peripheral muscle dysfunction, decreased activity levels and nutritional abnormalities, all of which contribute to poor functional status, decreased quality of life and increased health care utilization. Lack of recognition and understanding of the complex interactions of these common co-morbid conditions and systemic effects often contributes to sub-optimal care for patients with COPD. Comprehensive in covering the range of co-morbidities that often accompany COPD, Chronic Obstructive Pulmonary Disease: Co-Morbidities and Systemic Consequences provides a concise, evidence-based review of the problem and its systemic manifestations. All physicians interested in this growing disorder--from subspecialists to primary care providers--will find in this timely and informative title the practical, state-of-the art approaches needed to provide targeted, integrated care for patients with COPD.

Abstract
Sleep disorders represent a major portion of the chief complaints seen by pulmonologists and other physicians. Sleep apnea and hypopnea syndrome for example, are common disorders with significant adverse health consequences. Sleep apnea is associated with increased cardiovascular mortality, impaired quality of life and increased motor vehicle accidents. In addition, sleep apnea often co-exists with other chronic conditions including obesity, the metabolic syndrome, and tobacco use disorder. Patients with sleep-related conditions often present with non-specific complaints that require a broad and detailed knowledge of the wide range of sleep disorders. In Essentials of Sleep Medicine: An Approach for Clinical Pulmonology, a concise, evidence-based review of sleep medicine for the pulmonologist is presented. Providing a focused, scientific basis for the effects of sleep on human physiology, especially cardiac and respiratory physiology, chapters also outline a differential diagnosis for common sleep complaints and an evidence-based approach to diagnosis and management. This includes a review of the current standards of practice and of emerging technology and unresolved issues awaiting further research. Each chapter includes a summary of current research and outlines future research directions and issues. In all, Essentials of Sleep Medicine: An Approach for Clinical Pulmonology provides a clear diagnostic and management program for all the different sleep disorders, with a major focus on respiratory disorders of sleep, and includes key points and summaries. Developed by an international group of renowned authors, Essentials of Sleep Medicine: An Approach for Clinical Pulmonology is an invaluable resource for pulmonologists, respiratory care practitioners, polysomnographic technologists, graduate students, clinical researchers, and other health professionals seeking an in-depth review of sleep medicine.
