Abstract
This book presents a detailed and updated review of the widespread changes that take place during adolescence, adopting a preventive perspective that reflects physical, social, cognitive, and emotional changes. It addresses a broad range of aspects, including: the preventive programs and their systemic effects; the role of environment in influencing the healthy behaviors of adolescents and young adults; the use of e-Health technology in health and behavioral interventions for adolescents; and the clinical and prognostic implications of primordial prevention in healthy adolescents. All of these elements are subsequently reviewed using a multidimensional approach, in order to offer extensive information on the complex changes that characterize adolescents’ physiological, psychological, and neurobiological development. In addition, the book depicts the preventive strategies currently used in various social settings (school, family, sport club, health policies) aimed not only at reducing lifestyle risk behaviors, but also at improving resilience, happiness, social involvement, self-esteem, and sociability. This update is essential in the light of the fact that, to date, prevention has mainly been directed towards adolescents with physical or mental disorders rather than their healthy peers. As such, the book offers a valuable tool for pediatricians, child and adolescents psychiatrists, and for all professionals involved in Health Promotion and Disease Prevention.

Abstract
For over 30 years, the stepped care approach has been used in the management and treatment of hypertension. This approach advocates the use of initial monotherapy to reach the desired blood pressure, and the subsequent use of additional drugs with complementary modes of action, until the target BP is achieved. However, current control rates for hypertension suggest that such an approach may be outdated and no longer effective in the management of hypertension. Clinical trials have shown that more than 50% of patients require a combination of drug therapies to achieve their target BP. Some proponents of combination therapy have argued that the risk of adverse side effects and non-compliance will be lower than that compared to monotherapy, which initially requires higher doses to be administered to be effective. Consequently, there have been increasing calls of the necessity for a much more combined approach in the treatment of hypertension. Adding to this, Combination Therapy in Hypertension will cover current control rates of blood pressure worldwide, the benefits of monotherapy and combination therapy, examine the effectiveness, risk of adverse effects, compliance, and cost of the available therapies and will also include information on relevant outcome studies.
Contents
State of Hypertension Control -- Historical Review of Combination Therapy -- Advantages of Combination Therapy Compared with Monotherapy -- Use of Combination Therapy in Special Populations -- The Evidence Base for Combination Therapy -- Combination Therapy as First-Line Treatment -- Future Combination Agents -- Clinical Inertia in the Management of Hypertension.

Abstract
This guide brings thought leaders and master clinicians together to share their wisdom and expertise regarding clinical decisions surrounding unilateral and bilateral vocal fold paralysis. Designed as a what- to-do and why as opposed to a how-to guide, the authors detail the reasoning process from work up, through intervention, to post-treatment decision making. The text is organized around decision points in the management of vocal fold paralysis such as decisions in regard to timing, intervention, implants, reinnervation, drugs, voice therapy, and post-operative care. Individual chapters focus on specific steps in the process of evaluation and treatment, explore the decisions that can and should be made, and provide answers and direction for the reader. Authors explain the issues around the decision point and use their considerable experience to offer their opinion and as well as the thought process behind it. Decision Making in Vocal Fold Paralysis will be a useful guide for practitioners that deal with vocal fold paralysis, including otolaryngologists, general surgeons, neurologists, speech language pathologists, primary care physicians, and oncologists.

Abstract
Many children with medical conditions fail to improve despite physicians' best efforts. Sometimes, we ascribe this failure to lack of adherence to therapy, or to the severity of the condition. What we often fail to appreciate, however, is that sometimes the lack of improvement can be explained by the patients' psychological states. The first section of Functional Symptoms in Pediatric Disease: A Clinical Guide teaches children's health care providers to recognize functional symptoms that can complicate organic disease, as well as symptoms that are believed to be purely functional in origin. Literature reviews, case studies and quizzes are provided in each chapter, with video demonstrations included in some of the chapters. The second section of the book will help clinicians differentiate the patients for whom referral to a mental health provider is mandatory from those for whom other approaches may be useful. For the latter group, the book teaches clinicians to empower themselves by learning how to incorporate various therapies for functional disorders into their practice, including biofeedback, basic cognitive behavioral therapy techniques, hypnosis, acupressure, yoga and meditation. Practical strategies for obtaining training in these modalities are provided in the appendix. Useful for practicing clinicians including pediatricians, family practitioners, pediatric subspecialists, child psychiatrists, psychologists, other mental health care providers and practitioners of alternative and complementary medicine, Functional Symptoms in Pediatric Disease: A Clinical Guide is an important new book that will help children's health care providers consider the possible impact of functional contributions to the clinical presentation of their patients.

Abstract
The increasing prevalence of morbid obesity has led the World Health Organization to coin the descriptive term "globesity" to reflect the worldwide nature of the problem. Providing health care to these patients, especially when surgery is required, can be extremely challenging owing to the specific needs in respect of logistics, facilities, and professional expertise. Appropriate care has too often been unachievable and unaffordable outside of established bariatric centers, but such centers themselves usually have insufficient capacity and resources to cope with growing demand. This book aims to provide guidance and helpful tips and tricks on how to deal with obese patients within a general surgery setting. Epidemiology, organizational and logistical aspects, nursing issues, patient assessment, anesthesiology, and surgical practicalities are expertly covered in the opening chapters. Techniques of relevance to the general surgeon are described according to anatomic region, covering the head and neck; cardiothoracic and vascular system; upper and lower GI tract; pancreas, liver and adrenal glands; urinary tract and kidneys; the reproductive system; and the abdominal wall. Results achieved by bariatric surgery worldwide are reviewed, and the book closes with a chapter devoted to plastic and reconstructive surgery. The Globesity Challenge to General Surgery highlights a need for global rethinking on public health as regards resource allocation and patterns and standards of care, improving outcomes through greater affordability.

Abstract
This handy volume focuses on the evaluation and treatment of a wide range of sports injuries, in relation to the Tanner stage of sexual development in young athletes. Recognized leaders in each subspecialty illustrate their own treatment preferences, and a series of detailed chapters address the injuries likely to be encountered in different parts of the body, including the spine, shoulder, elbow, wrist and hand, hip, knee, ankle, and feet. Among the topics that receive particular attention are epiphyseal-physeal injuries, the still controversial issue of capsular ligament tears and the impact of their treatment on joint and bone development, and stress syndromes of the musculoskeletal system. Helpful algorithms covering the diagnosis and treatment of ligament tears are included, and advice is provided on when to return an athlete to training. One chapter is devoted to non-sports-related pathologies that have to be considered in the differential diagnosis of sports injuries in adolescents.

Abstract
Topics in pediatric orthopedics routinely surface in general pediatrics and primary care medicine, whether on board exams or in clinical practice. Musculoskeletal-related problems are common presenting complaints in any general pediatric or family medicine practice, and in emergency medicine. From birth injuries to musculoskeletal infections and sport-related injuries, many orthopedic conditions and presenting complaints must be addressed by the primary care physician or pediatrician. Pediatric Orthopedics: Handbook for Primary Care Physicians is a concise and targeted handbook filled with all need-to-know conditions, injuries, and diseases, supplemented by over three hundred photos and illustrations. It describes common orthopedic conditions, how to diagnose them, and the indications to either treat these conditions or refer the child to an orthopedist. Readers will learn the natural history of orthopedic diseases and how to differentiate between normal development and abnormal pathological conditions. An easy-to-read bulleted format highlights the most pertinent information for conditions commonly encountered in primary care. In addition, the book provides critical information used to answer musculoskeletal questions on the pediatrics board exam. The two editors come from different specialties (pediatric orthopedics and pediatrics) and present topics in a practical and problem-oriented method. Pediatric Orthopedics: Handbook for Primary Care Physicians is invaluable to all health providers taking care of children, including pediatricians, family medicine physicians, emergency medicine physicians, nurse practitioners, physical therapists, and medical students.

Abstract
Psychosocial problems appear within a medical context worldwide, and are a major burden to health. Psychosomatic Medicine: An InternationalPrimer for the Primary Care Setting takes a uniquely global approach in laying the foundations of biopsychosocial basic care (such as recognizing psychosocial and psychosomatic problems, basic counseling, and collaboration with mental health specialists) and provides relevant information about the most common mental and psychosomatic problems and disorders. The scope of the book is interculturalit addresses global cultures, subcultures living in a single country, and strengthening the care given by physicians working abroad. This clinically useful book outlines best practices for diagnosing the most common biopsychosocialproblems and mastering the most common communication challenges (e.g. doctor-patient conversation, breaking bad news, dealing with difficult patients, family and health systems communication and collaboration). Every chapter integrates basic theoretical background and practical skills and includes trans-culturally sensitive material, important for work with patients from different cultures. Psychosomatic Medicine: An InternationalPrimer for the Primary Care Setting serves as an excellent resource for clinicians hoping to gain and develop knowledge and skills in psychosomatic medicine.

Abstract
This book describes in detail the potential role of ICT and electronic systems, together with the application of Web 2.0 technologies, in telepediatrics and child health. Rather than simply proposing engineering solutions that may soon become outdated, it is designed to address those real needs that telemedicine and developers are asked to meet. The orientation of the book is very much toward primary care, and both low- and high-income settings as well as extreme or complex scenarios are considered. The first two sections of the book describe different fields of application, such as the community, the hospital, and children with chronic illnesses or special needs, and examine technical issues. The use of telemedicine in delivery of care in extreme rural settings and developing countries is then discussed, with attention also to major emergencies and humanitarian crises. The closing chapters consider the role of modern technologies in the education of caregivers who work with children. Child health is a crucial issue in both industrialized and developing countries. Telemedicine for Children's Health will be an excellent guide to the potential value of telemedicine devices in reducing the burden for children and parents and in offering quick and concrete solutions in low-resource scenarios.
