The book is written in a "cookbook format" that will enable plastic surgeons quickly to learn how to utilize the closed approach to rhinoplasty to optimal aesthetic benefit.

Nevertheless, the complete release of the skin from the nasal skeleton allows for more post-operative swelling, especially if the skeleton is greatly reduced. To enhance nasal definition many surgeons find it necessary to place grafts on the skeleton. This book describes a closed operation that avoids the unstable nose tip of past procedures by preserving the elastic tissue and greatly reduces post-operative swelling. It also presents a novel technique for tip refinement, which elongates the periphery of a dome-shaped tip and a new nose splint. This shape must comply with the patient’s demands, which, however, are often obscure, ambiguous, or inconsistent; thus even the greatest rhinoplasty surgeons have unhappy patients. The only appropriate approach is to suggest two or three shapes that suit the patient’s appearance and expectations, and ask the patient to choose between them. Of course, these suggestions must be precise, visualized, and specific. The technical difficulty, on the other hand, lies in assessing the anatomical character of the patient’s problem and selecting the procedure that will achieve the desired shape and the means to perform that procedure. This textbook is designed to help all readers: - Make accurate diagnoses by learning morphology based on varied anatomy - Simulate the target shape employing generally used retouch software - Suggest to patients different potential target shapes - Learn how to choose the appropriate procedure in order to produce the selected target shape It is the authors’ hope that readers will find this textbook an invaluable aid in the performance of rhinoplasties which are not merely based on the surgeon’s “taste” but rather take into account the individual patient’s requirements and achieve aesthetic results that guarantee patient satisfaction.

Abstract
This volume presents a novel logic-based, simplified understanding and approach to the external nose and face for aesthetics, airway, and sleep treatments that have mostly been under-recognized in the past. Key features of the text include an engineer’s approach to simple rhinoplasty, (Release, Resize, Reposition, Reinforce) instead of the typical step-wise procedure algorithm. It also includes a logical explanation of how facial skeletal anatomy is the true cause of sleep apnea and aesthetic deformity with the physics and evidence of how this works, and a review of conventional rhinoplasty/facial structural treatments, with an analysis of why these are flawed and need improvement. Finally, rhinoplasty surgical strategy is discussed using a Rhinoplasty Compass(TM) diagram. Rethinking Rhinoplasty and Facial Surgery appeals to the surgeon who has grown frustrated with the aesthetic and breathing results of the conventional treatment paradigm. It also appeals to the otolaryngologist, plastic or oral surgeon who avoids working on the external nose and nasal valve because of the perceived complexity of graft-style rhinoplasty or oversimplicity and limitations of basic septoplasty and turbinate reduction. This book provides a hybrid approach to the nose and face that provides more reliable and straightforward outcomes via an understanding of framework.

Practitioners will value the comprehensive coverage of the topic. The actual surgical steps involved in the various techniques form the main body of the work, while a wealth of intra-operative photographs facilitate the learning experience. The views expressed are solely those of the author, based on his personal experience. He makes no claims to the content being exhaustive or the only possible approach and has extensively referenced various experts in the field who have contributed to this topic. This book represents the author’s view of how to approach nasal tip deformities, and how to arrive at an algorithm in putting together various surgical techniques in a “sequence” to achieve meaningful results. The book also explores the clinical correlation, which is just as or arguably even more important in nasal tip surgery. The main chapters will focus on the suture, grafting, non-suture and non-grafting techniques in nasal tip reconstruction.
