Abstract
This book describes the approach to anesthetic and perioperative management in different categories of high-risk patient scheduled to undergo elective noncardiac surgery. Individual sections focus on patients with conditions entailing cardiac and hemodynamic risks, respiratory risks, renal and metabolic risks, neurological risks, and other forms of risk. For each condition, up-to-date guidance is provided on risk evaluation, risk stratification, and intraoperative and postoperative anesthetic management. This is the first time that such different clinical situations have been gathered together in a single textbook of anesthesiology. All of the authors are international experts with extensive clinical experience. The aim is to provide trainee anesthesiologists with clear guidance that will prove invaluable when managing a wide range of patients with serious illnesses or conditions that pose a significant anesthetic risk. This book proposes and investigates a universal framework, and accompanying documentation system, to facilitate and catalogue benefit-risk decisions; a valuable addition to the benefit-risk toolbox. Over the past decade, pharmaceutical companies and regulatory agencies have been reviewing the benefit-risk assessment of medicines with a view to developing a structured, systematic, standardized approach. Examining the evaluation of such an approach by several mature regulatory authorities ensures that the reader gains a unique insight into the ongoing debate in this area. The field of benefit-risk assessment continues to evolve at a rapid pace due to political and societal pressure, as is reflected in the recent FDA PUDFA agreement as well as in the EMA 2015 Roadmap. Rather than provide a comprehensive snap-shot of this constantly changing environment, this book evaluates selected current approaches to benefit-risk assessment. The strengths and weaknesses of publicly available documents in communicating benefit-risk decisions to stakeholders are reviewed, and these evaluations are used to inform development of a prospective framework that could be used to harmonise procedures globally.

At the core of this book lies the question how to approach medicines, risks and communication as a researcher - or anybody planning and evaluating a communication intervention, or wanting to understand communication events in private and the media. With a view to tackle current shortcomings of communication systems and processes for improved implementation, patient satisfaction and health outcomes, a multilayered approach is presented. This combines multiple data types and methods to obtain a wider and deeper understanding of the major parties and their interactions, as well as the healthcare, social and political contexts of information flows, how they interfere and which impact they have. Illustrated with real life experiences of safety concerns with medicines, worldwide active experts discuss the methods and contributions their disciplines can offer. With considerations on terminologies, tabulated overviews on communication types and outcomes, a patient-centred vision and plain language for non-medical readers, the book creates a platform for multidisciplinary collaborations amongst researchers as well as practitioners from communications, healthcare, the social sciences and pharmacovigilance. Importantly, it advocates for an active role of patients and highlights the achievements and aspirations of patient organisations. Finally, the book suggests establishing an inclusive discipline of humanities and epidemiology of medicinal product risk communication to realise full research potential. The book uses unique, visual presentations and case studies to explain the benefits of medical screening tests (e.g., mammography, prostate and colorectal cancer screening, cholesterol screening) and drugs (e.g., statins, VioxxäØ) and the risks associated with exposure to environmental contaminants (e.g., lead, dioxin, radon). This book will help patients and their families get more involved in making medical decisions, and citizens face critical questions about the environment. By putting the complexities of risk analysis in terms the general public can relate to, the authors are empowering people to make well-informed decisions. About the Authors: Erik Rifkin is the president of an environmental consulting firm that specializes in the characterization of ecological and human health risks from exposure to soil, water, air and sediments. His firm provides assistance and guidance to federal and state regulatory agencies and corporations regarding the nature and magnitude of environmental risks and potential remediation strategies. Dr. Rifkin's broad experience includes the communication of health risks and benefits to groups concerned with these issues. Edward J. Bouwer is Professor of Environmental Engineering at Johns Hopkins University in Baltimore, Maryland. He has extensive experience with water and soil pollution and treatment. His research provides guidance on defining and managing environmental risks and how to interpret human and ecological health risk data. Dr. Bouwer has served on several National Research Council committees that provide guidance on managing human and ecological risk Guest Author Bob Sheff, MD, received his medical training as a radiologist at UCLA and Johns Hopkins Medical Center. Prevention of prostate cancer and discrimination between aggressive and indolent forms are important clinical goals, and the acquisition of significant new evidence on means of achieving these aims makes this book particularly timely. A wide range of topics are covered by leading authorities in the field. The biology and natural history of prostate cancer are reviewed, and the role of lifestyle and dietary factors, assessed. Detailed attention is paid to risk prediction biomarkers and to the role of novel high-throughput nucleic acid-based technologies in improving risk prediction and thereby allowing tailored approaches to cancer prevention. Potential means of chemoprevention of prostate cancer are also reviewed in depth, covering the very positive new data on the impact of aspirin as well as evidence regarding 5-reductase inhibitors, DFMO, and lycopene. Guidance is provided on the differentiation of aggressive from indolent disease, and the policy and research implications of recent findings are examined.
