Abstract
The axis (second cervical) vertebra is of special interest owing to its particular anatomy, biomechanics, and position in the spine. Despite this, the role of the axis in the function of the cervical spine and the nature of its involvement in trauma and other pathological conditions are still not completely understood. This book covers all aspects of the axis vertebra and its disorders. Embryologic development, normal anatomy, and biomechanics of the axis and upper cervical spine are first discussed, and imaging appearances explained with the aid of standard radiographs and images obtained using advanced techniques. Congenital anomalies, fractures, infections, and tumors (benign and malignant) are then discussed in depth in individual sections. Detailed attention is paid to the different types of fracture and ligamentous injury, and the relevant surgical approaches are described. The Axis Vertebra is based on the personal experience and expertise of the contributing authors, enhanced by up-to-date information drawn from the literature. It will appeal to a range of practitioners, including orthopaedic surgeons, neurosurgeons, radiologists, physical medicine and rehabilitation doctors, and physical therapists.

Abstract
This unique and richly illustrated volume presents the state of the art in the comprehensive management of major neurosurgical diseases in the elderly (aged 65 and over). It explores all of the common neurosurgical pathologies affecting elderly patients, and emphasizes the paramount importance of tailored management strategies for quality of life. It highlights updated techniques for anaesthesia and critical care, as well as minimally invasive neurosurgical methods intended for this specific group of patients. Radiosurgery treatment is also discussed, in particular for brain tumours. In western societies, the proportion of elderly citizens has nearly reached 20%, and shows no signs of slowing down. The management of neurosurgical conditions in this particular population requires specific multidisciplinary strategies. To address this situation, a team of internationally respected contributors accurately describe degenerative and traumatic spinal diseases, which account for the majority of admissions among the elderly, as well as brain tumours and intracranial haemorrhages, aspects that are raising new ethical issues. The book mainly addresses the needs of neurosurgeons and geriatric neurologists, but also neuro-oncologists and neuro-anaesthesists working with elderly patients, as well as students in these disciplines.

The opening sections describe pretreatment images in various conditions, including trauma, degenerative disc disease, and osteoporosis, and the different types of neurosurgical and interventional treatment that may be used. The post-treatment appearances of normal sequelae and complications on conventional radiography, CT, and MRI are then documented in detail on the basis of a large series of clinical cases, with a wealth of images. Guidance is provided on selection of one or a combination of imaging modalities. This book will be an invaluable clinical and research tool not only for neuroradiologists but also for neurosurgeons, and interventional radiologists.

Considerations for team physician management comprises part I, including on-field assessment of spine injuries and concussion, rehabilitation and return to play, and complications and post-concussion sequelae. The second and third sections discuss injuries to the cervical spine and the thoracolumbar spine, respectively, describing injury evaluation, management, and outcomes in the elite athlete. Spine conditions have been studied extensively in the general population however, applying this data to the elite athlete is controversial. Numerous external variables make performance of well-designed clinical trials challenging in this population, and consequently evidence-based recommendations are lacking for the athlete’s spine.
