Abstract
Both surgical skill and histological preparation and evaluation are important for successful treatment of various malignant skin tumors. In addition, good knowledge of local patterns of infiltration is required to achieve complete excision. All tumors may show an irregular, often asymmetric, mainly horizontal subclinical infiltration with a wide extent. When deep infiltrations are present, they are also often asymmetric and hard to determine before excision. Until recently these tumor outgrowths have only been detectable using histologic evaluation, but 3D histology is highly sensitive and can detect even very small tumor infiltrations. Very narrow margins can thus be taken for excision, which helps spare healthy skin as only a subsequently targeted re-excision will be necessary to remove all of the asymmetric tumor infiltrations. Therefore the resulting defect will be as small as possible and as large as necessary, and can be adapted to the requirements for aesthetic surgical defect closure. 3D Histology Evaluation of Dermatologic Surgery compares the most commonly used methods in order to give the reader a graphical overview of the current state-of-the-art. The basic principles of various surgical methods and histological workups of the complete 3D tumor margins are described and extensively illustrated to allow better understanding. Furthermore, more complex treatments of larger tumors are also included. Therefore, this book will be vital for all involved in the management of these patients, including practicing and trainee dermatologists, ENT, plastic and facial-surgeons, pathologists and oncologists.

This book provides step-by-step instructions on how to operate with 27G instrumentation in a wide range of surgical indications, including vitreous floaters, macular holes, dropped nucleus, retinal detachment, diabetic retinopathy, submacular hemorrhage, retinopathy of prematurity and much trauma. All surgeries are approached in cookbook fashion, with initial coverage of the "ingredients" (devices and instruments) and then meticulous description of preparation and performance with supporting photographs, drawings, and videos. In addition, the differences and benefits in comparison with 23G and 25G vitrectomy are highlighted. Small-gauge vitrectomy has radically changed the ways in which vitrectomy is performed. 27G vitrectomy is the most recent and most exciting development in small-gauge vitrectomy. The healing time is faster but the small diameter prolongs the duration of surgery. This obstacle has been overcome with powerful new vitrectomy machines and revolutionary vitreous cutters with two cutting blades making 27G vitrectomy as fast as 25G. This new equipment has expanded the indications for 27G surgeries immensely from detachment surgery to retinopathy of prematurity and the advent of new instruments may make 27G the gold standard in the future. A detailed approach to the various scanning methods and image interpretation is demonstrated with illustrations, images, and video. The second section focuses on ultrasound anatomy of the abdomen. Each chapter demonstrates the normal and abnormal ultrasound anatomy of specific abdominal organ or organ systems (abdominal wall, liver, pancreas, biliary, stomach, anorectum, retroperitoneum, vascular) with illustrations and images. The current status and technique of endoscopic ultrasound of the upper and lower gastrointestinal tracks is addressed in this section. The third section provides a state-of-the-art review of the status of each major application of abdominal ultrasound in surgery. Topics range from trauma ultrasound and laparoscopic staging to techniques in ultrasound guidance and three-dimensional targeting. This section provides a comprehensive review of the employment of interventional, intraoperative, laparoscopic, transabdominal and endoscopic ultrasound. This book presents different patient-oriented perspectives from surgeons, economic evaluation and management researchers, and business companies active in the healthcare sector, striking a balance between the appropriateness/effectiveness of treatment and efficiency/cost. It does not include technical surgical details, but instead provides the necessary knowledge regarding different groups of patients to help economic and management researchers make accurate evaluations. Although partially based on the specific case of abdominal wall surgery in the Italian health system, the book defines a model that can, with the necessary adaptations, be applied in other national contexts. It also analyzes different reimbursement systems and methods of data collection. This approach supports the evolution form evidence-based medicine (EBM) to the future of real-world data (big data analysis). Further, it highlights the critical issue of “silos” reimbursement, which is the pillar of DRG, and proposes methodology to evaluate the direct and indirect benefit and costs of surgery (for example quality of care, costs incurred in cases of surgical complications due to the use of inappropriate, low-cost material or due to surgical procedure. It is a valuable resource for clinicians, surgeons, policymakers and managers in the field.
