Abstract
This volume presents the MD Anderson experience in providing care and services to the rapidly growing population of cancer survivors, which is currently estimated to be 12 million in the United States and more than 25 million worldwide. As cancer survival rates have increased, it has slowly become clear that the challenges faced by people with cancer do not end with treatment but simply change. This book aims to assist community oncologists, physicians, and their staff, who care for the vast majority of cancer survivors, by disseminating models of surveillance for disease recurrence, screening for second primary cancers, education regarding potential late effects of treatment, and psychosocial counseling. These models have proven valuable to cancer survivors who receive care at MD Anderson.

Abstract
This book discusses the impact on women of the diagnosis and treatment of early breast cancer. Readers will learn about the risks of breast cancer recurrence and interventions to reduce these, such as endocrine therapy and bisphosphonate therapy, as well as the role of lifestyle factors such as diet and exercise. The long-term effects of treatment on fertility, the heart and other systems are discussed, as well the psychological burden for women who are increasingly likely to be cured from their cancer. Each chapter provides background and a practical guide in the management of women after the initial phase of diagnosis and treatment. Authored by a multidisciplinary team this book provides all the relevant expertise as well as different perspectives, providing a holistic picture of early breast cancer consequences. From oncologists to general practitioners as well as specialist nurses, gynaecologists, psychologists and other healthcare professionals involved in the long-term care of women with breast cancer, this book provides a timely and indispensable guide to practitioners caring for early breast cancer survivors.

Abstract
This book addresses the growing problem of multimorbidity in cancer patients and survivors with the focus on how to best integrate the effective cancer care with the care of multiple chronic conditions. As cancer is more prevalent in older individuals, many patients with cancer also suffer from other chronic conditions that impact on the uptake, tolerance and outcomes of cancer treatment and their long term mortality and morbidity. In addition, cancer and its treatment increase the risk of future chronic conditions. Readers will examine the prevalence and predictors of chronic conditions in cancer, impact of chronic conditions on screening and treatment, evidence for preventative strategies that address both cancer and chronic conditions, emerging management and care integration strategies and directions for management of multimorbidity in special cancer populations: the very young, the very old and those at the end of life. Authored by clinicians and researchers from diverse expertise including epidemiology, sociology, hematology, medical oncology, palliative care, pharmacy and representing Australia, New Zealand, US, Canada and the Netherlands, the book brings an international perspective to a problem that affects all cancer settings. The book is going to be of interest to diverse professionals interested in cancer control including epidemiologists, public health researchers, policy makers as well as clinicians dealing with cancer patients within specialist cancer and non-cancer and primary care settings.
Contents
What is comorbidity? -- How do we measure comorbidity? -- Cancer, chronic conditions and social disadvantage: the perfect storm -- Impact of comorbidity on cancer screening and diagnosis -- Impact of comorbidity on treatment decision making and outcomes -- Impact of cancer and chronic conditions on caregivers and family members -- Prevention of chronic conditions and cancer -- Chronic condition management models for cancer care and survivorship -- The management of polypharmacy in people with cancer and chronic conditions -- Breaking the silos: integrated care for cancer and chronic conditions -- Advocacy in cancer and chronic conditions: challenges and opportunities -- Research considerations in patients with cancer and comorbidity -- The cost of cure: chronic conditions in Survivors of child, adolescent, and young adult cancers -- chronic conditions and cancer in older adults -- chronic conditions and cancer at the end of life.

Abstract
The timely revision of this authoritative handbook gives a wide range of health care providers practical insights and strategies for treating cancer survivors' long-term physical and mental health issues and promoting healthy lifestyle habits. Details of new and emerging trends in research and practice enhance readers' awareness of issues faced by survivors so they may better detect, monitor, intervene in, and if possible prevent distressing conditions and potentially harmful outcomes. Of particular emphasis are different models of care that may be applicable to diverse clinical settings around the world. New or updated chapters cover major challenges to survivors' quality of life and options for health care delivery across key life domains, including: Adaptation and coping post-treatment. Problems of aging in survivorship, including disparities and financial hardship. Well-being concerns including physical activity, weight loss, nutrition, and smoking cessation. Core functional areas such as work, sleep, relationships, and cognition. Large-scale symptoms including pain, distress, and fatigue. Models of care including primary care and comprehensive cancer center. International perspectives PLUS, insights about lessons learned and challenges ahead. With survivorship and survivorship care becoming an ever more important part of the clinical landscape, the Second Edition of the Handbook of Cancer Survivorship is an essential reference for oncology specialists, primary care providers, mental health professionals, rehabilitation providers, as well as public health specialists, epidemiologists and policy makers.

Abstract
This comprehensive guide describes the aftercare that is appropriate in young cancer patients and discusses in detail the risk and detection of treatment sequelae. It explains the impacts on body and mind of both the disease itself and the different risk-adapted cancer treatments currently in use. Clear guidance is provided on diagnosis and management of the principal treatment-related toxicities in different organs and organ systems and for a wide variety of tumor types. In addition, the role of genetic polymorphisms in the development of adverse therapy-related outcomes is explored, and advice offered on genetic counselling. As the number of long-term survivors of childhood cancer and of cancer in young adults continues to grow, so issues surrounding potential sequelae, second malignancies, and quality of life are becoming ever more important. All practitioners involved in the care of young cancer patients will find this book to be a helpful source of up-to-date information and assistance.
