Abstract
This timely text examines the causes and consequences of population displacement related to climate change in the recent past, the present, and the near future. First and foremost, this book includes an examination of patterns of population displacement that have occurred or are currently underway. Second, the book introduces a three-tier framework for both understanding and responding to the public health impacts of climate-related population displacement. It illustrates the interrelations between impacts on the larger physical and social environment that precipitates and results from population displacement and the social and health impacts of climate-related migration. Third, the book contains first-hand accounts of climate-related population displacement and its consequences, in addition to reviews of demographic data and reviews of existing literature on the subject. Although the book is largely written from the perspective of a researcher, it reflects the perspectives of practitioners and policymakers on the need for developing policies, programs, and interventions to address the growing numbers of individuals, families, and communities that have been displaced as a result of short- and long-term environmental disasters. Global Climate Change, Population Displacement, and Public Health is a vital resource for an international audience of researchers, practitioners, and policymakers representing a variety of disciplines, including public health, public policy, social work, urban development, climate and environmental science, engineering, and medicine.

Abstract
Intensive care medicine is one of the fastest growing services provided by hospitals and perhaps one of the most expensive. Yet in response to the global financial crisis of the last few years, healthcare funding is slowing or decreasing throughout the world. How we manage health care resources in the intensive care unit (ICU) now and in a future that promises only greater cost constraints is the subject of this book, the third in an informal series of volumes providing a global perspective on difficult issues arising in the ICU. Leading healthcare experts, including critical care physicians, critical care nurses, ethicists, and attorneys, provide snapshots of current ICU resource allocation in 12 developed countries on which other experts then draw to analyze resource allocation and consumer demand at the level of the global medical village. The process is repeated with an eye toward the future that takes into account initiatives and reforms now underway. A fictional healthcare plan, the "Fair & Equitable Healthcare Plan," is put forth to address weaknesses in existing approaches, and healthcare experts and ethicists are invited to respond to its often provocative provisions. Structured as a dialogue, the book is a great starting point for serious discussion about the looming issue of ICU healthcare resource allocation.

Abstract
The goals of Integrated Care: Working at the Interface of Primary Care and Behavioral Health are to educate psychiatrists about the fundamental shift underway in health care and to prepare them to be successful and effective in the new health care arena. The passage and implementation of the Patient Protection and Affordable Care Act presents an opportunity for newly insured patients and for funding models of integrated care, enabling psychiatrists to have a more significant population-level impact. The only book of its kind, the guide defines integrated care, reviews the evidence base, and addresses the three potential benefits of this model of care: improved outcomes, cost containment, and enhanced patient experience (also known as the "triple aim"). The new models of integrated care presented in this book are population-based, which is the key to improved outcomes, and they represent a change in how medicine in general and psychiatry in particular will approach health care delivery moving forward.

Abstract
This book provides up-to-date information on lymphatic filariasis supported with abundant images, tables and algorithms. It is a first such monograph on a disease that has varied presentations which are complete clinical entities, such as chyluria, hydrocele, elephantiasis, etc. This book consists of three parts, parasitology, acute clinical manifestations and chronic presentations of the disease. Approximately 120 million people are infected with lymphatic filariasis in tropical and subtropical countries. About a third of these suffer clinical consequences of this infection, and many are seriously disfigured. India, Indonesia and Nigeria are the most endemic countries in the world. In spite of Global Control Program of WHO, filariasis continues to be a significant medical challenge and current efforts underway will take a long time to bring it under control. Lymphatic Filariasis is meant for general surgeons, physicians, urologists and plastic surgeons, including the postgraduate students. Useful for epidemiologists, internists, and those involved in filariasis prevention programs.

Abstract
This book describes the challenges involved in developing mTOR inhibitors for cancer treatment, starting with an in-depth examination of their molecular mechanism of action, with emphasis on the class side-effects, efficacy and mechanisms of resistance, as well as on promising novel directions for their development, including novel compounds and rational combinations with other anti-neoplastic drugs. Over the last 10 years, inhibitors of mTOR have emerged as a major class of anticancer drugs. Two rapamycin analogs are currently approved for the treatment of renal cell carcinoma, and it is estimated that a variety of other tumor types could benefit from mTOR inhibition, with numerous clinical trials (including pivotal registration trials) already underway. Second-generation small-molecule inhibitors of the pathway have also shown promise in terms of their superior tolerability and efficacy and are undergoing extensive clinical evaluation, with an estimated 30+ compounds currently under evaluation.

Abstract
This open access book about the Zadeh Project demonstrates and explores a core question in clinical ethics: how can ethics consultants be accountable in the face of a robust plurality of ethical standpoints, especially those that underwrite practices and methods for doing ethics consultation as well as those viewpoints and values encountered in daily clinical ethics practice? Underscoring this question is the recognition that the field of clinical ethics consultation has arrived at a crucial point in its maturation. Many efforts are underway to more formally “professionalize” the field, with most aimed toward stabilizing a specific set of institutional considerations. Stretched between these institutional and practical initiatives resides a crucial set of of ethical considerations, chief among them the meaning and scope of responsibility for clinical ethics consultants. Developed around a long-form case scenario, the Zadeh Project provides a multi-layered series of “peer-reviews”: critique of the actions of the case scenario’s ethics consultant; reflection on clinical ethics method; examination of the many ways that commitments to method and practice can, and do, intersect, overlap, and alter one another. The design and format of this book thus models a key element for clinical ethics practice: the need and ability to provide careful and thoughtful explanation of core moral considerations that emerge among diverse standpoints. Specifically designed for those studying to become and those who are ethics consultants, this book, with its innovative and multi-layered approach, allows readers to share a peer-review-like experience that shows accountability to be what it is, an ethical, not merely procedural or administrative, undertaking.

Abstract
Transcatheter aortic valve implantation and percutaneous mitral valve repair have now been demonstrated to be credible alternatives to surgery in high-risk patients. The interest in these techniques is such that further significant technological changes are expected over the next few years. This revolution is already underway, rendering much prior knowledge obsolete. The success of the first edition of Percutaneous Treatment of Left Side Cardiac Valves has convinced us of the need to update the book in order to keep pace with the rapid and dynamic evolution in the discipline. Once again, this practical guide is designed to provide the reader with complete state of the art information on the techniques and approaches to percutaneous treatment of left side cardiac valve disease. Numerous images will help the reader to understand in detail the steps of each procedure. The potential complications and expected or potential morbidity from each procedure are discussed in depth, and the best ways to manage them are carefully explained. The book is also intended as a reference covering the up-to-date knowledge contained in the literature on the application of transcatheter techniques to cardiac valves.

Abstract
Since its introduction about sixty years ago, stereotactic and functional neurosurgery has evolved into a fascinating and interdisciplinary endeavor that combines modern neurosurgery, neurobiology, and neuroimaging with innovative diagnostic and treatment strategies. In this collection, acknowledged experts from Europe and North and South America present their scientific and clinical experience in stereotactic and functional neurosurgery for movement disorders and brain tumors. The contributions present a wide range from the beginnings of human stereotactic neurosurgery to the most modern molecular and restorative strategies to treat diseases of the human nervous system. They clearly demonstrate that the discipline is still young and dynamic with alternative and sometimes competing strategies whose evaluation is underway. They also document that operative lesioning techniques such as thalamotomies, though still chosen under certain circumstances, have been succeeded by novel neuromodulation techniques such as deep brain stimulation in the great majority of clinical cases.
