A growing majority of women in the western hemisphere have been delaying initiation of childbearing to later in life. Consequently, more women in their late 30s to early 40s are attempting to get pregnant for the first time than ever before. Since the incidence of most cancers increases with age, delayed childbearing results in more female cancer survivors interested in fertility preservation. In this book, the editors provide strategies for fertility preservation in women and men who require gonadotoxic treatment. In addition, epidemiologic, ethical, medico-legal, psychologic, and social aspects of fertility preservation are discussed. Using a format that combines concise scientific background with practical methodological information and easy-to-grasp algorithms, the chapters all conform to a uniform structure, including a brief abstract, keyword glossary, step by step protocol of laboratory procedures, key issues in commentary and a list of references. Abstract
Diffusion MRI allows indirect estimation of the perfusion and cellularity of normal and abnormal tissues and has conventionally been used in the brain in order to evaluate ischemia, tumors, and other conditions. Application in other parts of the body has been hampered by the occurrence of motion and susceptibility artifacts. However, recent advances in MR technology are overcoming these hurdles, allowing the use of diffusion MRI in organs outside the brain. In this book, the authors present cases drawn from daily clinical practice to illustrate the role of diffusion sequences, along with other morphological and functional MRI information, in the work-up of a variety of frequently encountered oncological and non-oncological diseases. Breast, musculoskeletal, whole-body, and other applications are covered in detail, with careful explanation of the pros and cons of diffusion MRI in each circumstance. Quantification and post-processing are discussed, and advice is provided on how to acquire state of the art images, and avoid artifacts, when using 1.5- and 3-T magnets. Applications likely to emerge in the near future, such as for screening, are also reviewed. The practical approach adopted by the authors, combined with the wealth of high-quality illustrations, ensures that this book will be of great value to practitioners.

The advice regarding report preparation is strongly supported by informative PET, CT, and PET-CT fused images of each disease. The book is broadly structured according to anatomic region, and a wide range of common diseases likely to be imaged using PET-CT is covered. This book will be of value to all those training or working in the field of oncology who wish to ensure that they are best placed to contextualize, interpret, and report the findings obtained with PET-CT, which can have such a dramatic impact on prognosis, therapeutic choice, and quality of life.

Abstract
The literature on the late effects of cancer treatment is widely scattered in different journals since all major organ systems are affected and management is based on a variety of medical and surgical treatments. The aim of ALERT Adverse Late Effects of Cancer Treatment is to offer a coherent multidisciplinary approach to the care of cancer survivors. The central paradigm is that cytotoxic multimodal therapy results in a perpetual cascade of events that affects each major organ system differently and is expressed continually over time. Essentially, radiation and chemotherapy are intense biologic modifiers that allow for cancer cure and cancer survivorship but accelerate senescence of normal tissues and increase the incidence of age-related diseases and second malignant tumors. Volume 2 of this two-volume work comprehensively documents potential late effects in all the normal tissue anatomic sites in the human body. The detection, diagnosis, management and prevention of effects are all considered in detail, and prognostic outcomes are discussed. Radiation risk factors and interactions with chemotherapy effects are clearly presented. The text is accompanied by numerous supportive illustrations and tables.
