Abstract

Step by step presentations of the most advanced MISS (endoscopy) and standard procedures such as ACDF (anterior cervical decompression and fusion), using cages or autologous bone, posterior approach to axis instability, minimally invasive stabilization systems, and cervical disc arthroplasty are examined for the treatment of the osteodiscoarthrosis. Looking towards the future, this book also includes chapters about the role of materials in cervical spine fusion and biomechanical engineering evaluation in cervical tribology. Considering the growing incidence of cervical spine discoarthrosis, the authors aim to provide an expert overview of cervical spine surgery to assist the medical community with treating this increasingly common pathology.

Abstract
This book presents results obtained from the whole mount preparations, radiological, and histological studies of 60 pu/pu and pu/+ mice from late embryo until 3 months of age. Most mice were in the embryo to 6 week age group where vertebral developmental changes are most marked. Although vertebral abnormalities have been identified as due to mutations in the delta-like 3 (Dll3) gene, it is evident that each mouse has differing structural abnormalities. The disorder is analogous to human congenital scoliosis, a common variant of which is spondylocostal dysplasia. The histological studies presented in this book include plastic embedded sections which allow for high level resolution not only of vertebrae, intervertebral discs, and ribs but also of associated spinal cord, nerve roots and ganglia. In addition an overview of embryo and neonatal development in mouse, chick and human vertebrae is provided to better assess how and where deviant pathoanatomy occurs. The book discusses the possible variables involved in creating final deformity beyond the gene abnormality itself.

The intervertebral disc is composed of a complex tissue that separates neighboring vertebrae, permits a wide range of motion, and cushions the high biomechanical forces on the spine. Disc degeneration leads to a loss of function and is often associated with excruciating pain. Written by leading scientists and clinicians, this is the only book in the past fifty years devoted entirely to the study of the intervertebral disc. The first part of the book provides a review of the basic biology of the disc in health and disease. The second part considers strategies for mitigating the effects of disc degeneration and discusses the possibility of engineering replacement tissues. It causes the spine to look like the letter ‘S’ or ‘C’ instead of a straight ‘I’. In scoliosis, the spine’s vertebrae may also be rotated, causing one shoulder blade or trunk muscle to be more prominent than the other.

Abstract

This issue of Acta Neurochirururgica presents the latest surgical and experimental approaches to the craniovertebral junction (CVJ). It discusses anterior midline (transoral transnasal), posterior (CVJ craniectomy laminectomy, laminotomy, instrumentation and fusion), posterolateral (far lateral) and anterolateral (extreme lateral) approaches using state-of-the-art supporting tools. It especially highlights open surgery, microsurgical techniques, neuronavigation, the O-arm system, intraoperative MR, neuromonitoring and endoscopy. Endoscopy represents a useful complement to the standard microsurgical approach to the anterior CVJ: it can be used transnasally, transorally and transcervically; and it provides information for better decompression without the need for soft palate splitting, hard palate resection, or extended maxillotomy. While neuronavigation allows improved orientation in the surgical field, intraoperative fluoroscopy helps to recognize residual compression. Under normal anatomic conditions, there are virtually no surgical limitations to endoscopically assisted CVJ and this issue provides valuable information for the new generation of surgeons involved in this complex and challenging field of neurosurgery.
