Abstract
Neglected finger injuries may lead to chronic lesions that often have detrimental consequences for the practice of a ball sport, whether at a recreational level or at the top athlete level. The initial injury management is crucial. Depending on the type and the severity of the lesions, different people may get involved: the players themselves or their entourage, physiotherapists, club doctors, emergency physicians, family doctors, sports doctors or hand surgeons. Treatment is usually conservative: the long fingers require rapid mobilization to prevent stiffness and contrary to that, the thumb requires stability. Surgery may be necessary to reach these goals especially for athletes, because of the demands of their sport. Chronic lesions are also in part related to repeated trauma, requiring specialized long-term multidisciplinary treatment. They can often lead to the end of a sporting career, but also may limit the functionality of the fingers at the time of conversion. Abstract
This is the first volume in the new Springer Major Reference Work series entitled Coloproctology. The book covers key topics in the anatomy and physiology of the colon, rectum and anus and the diagnosis of colorectal/anal diseases and disorders. Global Health and Volunteering: A Guide for Healthcare Professionals is designed to educate volunteers to be effective partners in delivering medical services locally and globally. Healthcare professionals are increasingly interested in global health and volunteering in areas of acute need. The biggest challenge to health in many locales is the inability to access the health care system. When people do connect with medical services, medications and surgical opportunities for chronic disease (i.e. glaucoma, diabetes, or hypertension) are often not affordable or cannot be sustained for a long period of time. The contributions in this book focus on a respectful dialog with local people and a willingness to learn from new experiences on the part of the volunteer. Skills transfer from visiting personnel to local providers is featured as a means to enhance healthcare sustainability. An appreciation of differing cultures, an understanding of the local economic conditions and challenges, and strategies for collaborating with the existing medical establishment are foundations of successful volunteer experiences as highlighted in this book. Dimensions of global health such as professionalism, religious beliefs, ethical dilemmas, traditional medicine, and alternative strategies for service are addressed by experts. Written and edited by leaders in the field, many of whom have more than two decades of experience volunteering abroad, Global Health and Volunteering: A Guide for Healthcare Professionals imparts lessons learned to help the reader avoid initial mistakes, while making the global health commitment stronger.

This book discusses globalization and its impact on human health. The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. These changes produce complex and constantly varying interactions between the biosphere, economy, climate and human health, forcing us to approach future global health trends from a new perspective. Preventive actions to improve health, especially in low-income countries, are essential if our future is going to be a sustainable one. After a period of undeniable improvement in the health of the world's population, this improvement is likely to slow down and we will experience--at least locally--crises of the same magnitude as have been observed in financial markets since 2009. There is instability in health systems, which will worsen if preventive and buffering mechanisms do not take on a central role. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of health as a common good. An entirely competition-driven economy cannot--by its very nature--address global challenges that require full international cooperation. A communal global leadership is called for. Paolo Vineis is Chair of Environmental Epidemiology at Imperial College. His current research activities focus on examining biomarkers of disease risk as well as studying the effects of climate change on non-communicable diseases. This book serves as a roadmap for the development and application of patient-reported outcome (PRO) measures, supporting beginners through to experts, as a practical guide. To elucidate on key concepts in the book, examples from clinical research in hyperhidrosis and health-related quality of life and medicines clinical development context, are used. Health-related quality of life represents one of the most commonly measured PROs in both routine clinical practice and research. The book demonstrates the importance of PROs to patients with chronic disease and how such outcomes can assist clinicians in managing patients and monitoring their response to treatment in terms of both symptoms and impacts. This book will benefit readers as a single-source practical guide on the development of modern PRO measures and may also serve as a blueprint for the conceptualization and planning of evidence generation related to PROs in various settings. Ideas and suggestions on how to navigate recent developments shaping the field of PRO measurement are also offered. Dr Paul Kamudoni (PhD MSc.) is a Scientific Associate Director in the Global Evidence and Value Development—R&D department at Merck KgaA in Darmstadt (Germany) where he leads the Centre of Excellence for Patient Reported Outcomes. Dr Kamudoni trained as a Pharmacoeconomist and a Clinical Outcome Measurement Scientist at Cardiff University. In his day to day work, he is involved in the development and validation of PRO measures, and the design and integration of PRO endpoints in clinical trials and other types of studies. He maintains an active research interest in methodological issues in the integration of electronic health records with prospective PRO assessments, and the application of item response theory in PRO measurement among others. Dr Nutjaree Pratheepawanit Johns (PhD PharmD) is an Associate Professor at the Faculty of Pharmaceutical Sciences, and melatonin research group, Khon Kaen University, Thailand. She has held a number of academic positions both in Thailand and abroad, including a fellowship at the Western Institutional Review Board (WIRB) (Olympia, WA, USA). Dr Johns has an expertise in cancer care and quality of life (QoL) research and collaborates with several cancer centres in Northeastern Thailand. She has developed and tested a number of PRO measures including cross-cultural adaptation into the Thai language. Dr Johns is also a principal investigator in many clinical trials investigating the effects of disease or interventions on QoL of cancer patients. She has been a recipient of several prestigious awards including Scholarship for Developing Country, International Society for Quality of Life Research and the Doyle Prize: Postgraduate section, Palliative Medicine Journal. Professor Sam Salek (PhD RPh FFPM FRPS MCMS FESCP) is Professor of Pharmacoepidemiology at the University of Hertfordshire, Hatfield, visiting Professor at the Estate of Hessen, Germany, and Director of the Institute for Medicines Development, Cardiff, UK. Professor Salek's research focuses mainly on four areas: development and application of PRO measures; drug safety evaluation and pharmacovigilance; pharmacoeconomics and economic evaluation of healthcare; and pharmaceutical regulation & medicines development in the mature and emerging markets. He has published 22 books and 600+ journal articles and abstracts. Professor Salek is a member of five Editorial Boards and an active member of several societies.
