Abstract
This book describes the surgical bariatric procedures most frequently performed worldwide and examines their evolution in recent years both within Italy and internationally. For each operation, indications, the surgical technique, potential complications, and the outcomes with respect to weight and obesity-associated comorbidities are presented. In view of the significant failure rate revealed by studies on the long-term results of bariatric surgery, the problem of weight regain and revision surgery are also discussed in detail, covering the different types of revision, conversion to other procedures, and the main outcomes. In addition, individual chapters focus on selected topics of importance. The role of bariatric surgery in the cure of type 2 diabetes ("diabetes surgery") is discussed and the debate over the significance of gastroesophageal reflux disease and hiatal hernia for choice of procedure is summarized. Finally, the most common endoluminal procedures, which have been gaining in importance, are described and other bariatric operations, outlined.

Abstract
This book, which describes in detail the most common procedures employed in inguinal hernia surgery, is exceptional in reflecting fully the most recent advances, as well as the most established techniques, in knowledge and practice. The role of recently developed and emerging technologies, including robotics, minimally invasive surgery, and biological meshes, is clearly explained, highlighting useful tips and tricks. The descriptions of traditional procedures, such as the Lichtenstein repair, are brought fully up to date and take into account the novel perceptions that professionals have of them, for example regarding the important role of nurses. The reader will also find guidance on established and new treatments for specific pain conditions, including postoperative chronic pain, the pubic inguinal pain syndrome, and obscure groin pain in women. The need for such an update on inguinal hernia surgery became clear at the 1st World Conference on Abdominal Wall Hernia Surgery, which highlighted the huge differences in the approaches adopted by scientists around the world and was chaired by the editor of this book. Inguinal Hernia Surgery will be a useful tool for all health care practitioners involved in hernia surgery.

Abstract
The treatment of liver diseases comprises a significant component of the practice of any general surgeon, whether working in an academic, a general, or a community hospital. With the rapid expansion in knowledge and technology, the liver has also been approached laparoscopically in specialized centers, with well-recognized advantages for patients. This book aims to communicate the large body of experience that has now been accumulated in minimally invasive liver surgery, with comprehensive and up-to-date information on the newest techniques. After a concise description of the main tools and technology necessary to carry out safe laparoscopic and/or robot-assisted liver surgery, individual operative techniques are explained and illustrated. Each chapter includes introductory information, a summary of indications and contraindications, and a detailed description of surgical procedure, including potential complications of both the surgery and the postoperative care. Special efforts have been taken to ensure that the illustrations are accurate and informative. The text is supplemented by a DVD of different procedures as performed by leading European liver surgeons.

Abstract
This book analyzes all aspects of modern multimodal treatment of locally recurrent rectal cancer in the pelvis in order to offer a panoramic overview of the different therapeutic options and enable the reader to balance their benefits and drawbacks. Indications for surgery and the surgical techniques themselves, including reconstruction techniques after major exenterative surgery, are clearly described and evaluated. The role of new imaging tools, such as MRI and PET-CT, in staging recurrences and guiding therapy is assessed, and detailed consideration is given to the use of neoadjuvant and iterative radiotherapy and the indications for systemic chemotherapy. Morbidity, mortality, oncologic outcomes, and quality of life issues are carefully analyzed, and options for pain control and palliation are examined. Finally, a national survey is presented on the state of the art in the surgical treatment of locally recurrent rectal cancer in Italy. While there has been a dramatic fall in the incidence of local pelvic recurrences, their treatment remains a major clinical challenge which requires a multidisciplinary approach and careful selection of the most appropriate strategy in each case. This book will aid practitioners in making the correct decisions in individual patients.

This book presents the most recent developments in oncologic breast surgery and takes full account of diagnostic, pathologic, and radiologic inputs. It is divided into three parts, the first of which discusses the premises underlying the modern surgical approach to breast cancer. The second part is devoted to what might be termed the conservative program, i.e., breast conservation and oncoplastic surgery, conservative mastectomy, and sentinel node biopsy and axillary dissection. The final part of the book covers different forms of surgery and other treatments in particular settings. Topics include plastic and reconstructive surgery, DCIS surgery, radio-guided surgery, adjuvant systemic therapy, intraoperative radiotherapy, and the role of surgery in locally advanced and metastatic disease. The detailed descriptions of techniques are accompanied by numerous high-quality illustrations. This book will be of value to both experienced practitioners and surgical trainees.

During recent decades, more than 100 surgical procedures have been proposed to treat pelvic organ prolapse, and surgeons are still searching for the ideal approach. Although generally accepted guidelines and algorithms are still lacking, careful preoperative work-up and patient selection can serve as a sound basis for tailored surgery. In this comprehensive book, leading experts from around the world provide a detailed, up-to-date overview of the diagnostic and surgical approaches employed in patients with prolapse of the middle or posterior pelvic floor compartment. Each surgical technique is explained step by step with the aid of instructive figures. Guidance is also included on the management of surgical complications and of recurrent disease aspects that are too frequently overlooked in the scientific literature. This book will prove essential reading for all who are interested in functional colorectal disorders of the pelvic floor and will represent a unique and invaluable source of knowledge for general surgeons, colorectal surgeons, and urogynecologists, whether in training or practice.

Abstract
This book gives an updated and detailed overview of diagnostic and therapeutic strategies for rectal cancer. Rectal cancer has become an increasing and serious disease in western countries, mainly linked to lifestyle and bad alimentation habits. This specific cancer is not only a threat to the lifespan of patients, but also has a heavy impact on quality of life, due to its implications in the fecal, urinary and sexual functions. Recent guidelines of prevention and early diagnosis, as well as current and future diagnostic and staging work-up, are here presented in a clear and convincing way, linking each topic to the therapeutic options arising from the staging and including a chapter on the molecular evaluation of the tumor. To complete the multifaceted approach to this particular pathology, a large part is devoted to the in-hospital care of rectal cancer patients, beginning from the fast-track procedures and the enhanced recovery systems, up to the in-depth description of the available of surgical techniques, including salvage situations, accidents, complications, and their treatment. A final chapter will be dedicated to the follow-up policy and its advantages.

Abstract
This book describes the current applications of the robotic system in general surgery, focusing on the technical aspects of the procedures most commonly performed by this means. It provides a comprehensive overview of the state of the art in robotic surgery and presents the most recently available evidence as documented in the literature. The opening chapters review robotic platforms, discuss the general advantages, limitations, and strategies of robotic surgery, and identify challenges and critical elements when setting up a robotic program. The full range of applications of the robotic system is then covered, encompassing thoracic, esophageal, gastric, hepatobiliopancreatic, splenic, colorectal, endocrine, and transplantation surgery. Detailed attention is also paid to innovative applications and future trends in the robotic platform, with inclusion of a special chapter on fluorescence imaging during robotic procedures. The book closes by considering aspects related to credentialing and privileging, such as mentoring, proctoring, and teaching modules for residents and fellows. Against the background of increasing acceptance of the robotic surgery system across the globe, this book will be invaluable for all general surgeons. There is no doubt that within general surgery the system will continue to flourish on account of its undoubted technical advantages, which minimize most of the intrinsic shortcomings of laparoscopy.

Abstract
This is the first time a book about laparoscopy in emergency abdominal surgery has been published. Numerous articles have been published in specific surgical journals, but, until now, there has not been a book that collates all the aspects of this little-known field. The aim of this volume is to achieve a complete and easy presentation of all the implications associated with laparoscopy in emergency abdominal surgery. The book should be a manual that can be easily consulted by digestive, general, and specialized surgeons, especially in an emergency. The authors' contributions are founded on evidence-based medicine, which give the book scientific credibility, but this is coupled with their experience of daily practice, which adds an important complementary dimension to evidence-based medicine. This is balanced by an emphasis on clarity and accessibility, because the ultimate aim of the book is educational. We hope this book will be frequently consulted, in depth, not only by specialist practitioners, but also by undergraduate students, new graduates and surgeons in training.
Contents
1. Diagnostic Laparoscopy in Abdominal Emergencies -- 2. Gastroduodenal Ulcer -- 3. Acute Cholecystitis -- 4. Acute Pancreatitis -- 5. Acute Appendicitis -- 6. Role of Laparoscopy in the Treatment of Acute Diverticulitis: an Evidence-Based Review -- 7. Adhesive Small Bowel Obstruction -- 8. Incarcerated Hernia -- 9. Abstract
Squamous cell carcinoma of the esophagus has a lower incidence than other gastrointestinal tract cancer; however, there are many difficult issued related to the choice of appropriate therapy for these patients. In recent years, the use of multimodal treatment has greatly improved the prognosis of patients affected by this disease but several aspects of the oncological and surgical management are still controversial. This book aims to give a homogeneous approach to esophageal cancer treatment, based on the author's lifetime experience in conjunction with exhaustive review of recent literature. In each chapter surgeons and oncologists will find appropriate tools for correct management of the diagnosis and therapy of this difficult disease. The book analyzes all the aspects of staging and treatment, such as modern diagnostic and staging techniques (MRI or PET-CT), endoscopic treatment of early cancer, new minimally invasive and open surgical techniques, and combined treatment. In addition, there are chapters dedicated to the postoperative course of these patients, as well as their quality of life, with indications for the correct follow-up and treatment of relapses.

Abstract
This monograph summarizes state of the art knowledge regarding peritoneal surface malignancies, with in-depth description of treatment options and the results achieved to date. It explores the most challenging problems on the basis of the authors' very extensive clinical experience and examines the most relevant clinical trials. A comprehensive summary is provided of all phase 2 studies (the only available completed studies) and of ongoing and future phase 3 studies. Particular attention is paid to the results of integrated treatment comprising cytoreduction (peritonectomy) and hyperthermic intraperitoneal chemotherapy (HIPEC). Helpful background information is also included on the definition and clinical assessment of each clinical form. The book, drawing on data from the entire Italian experience as well as world literature, will be an outstanding benchmark for health professionals and researchers.
